摘要
医养结合机构,收住的多为半失能/失能、失智、慢性病共患的高龄老人,在面临重大传染性疾病危险时,因老年人感染后重症率及死亡率高的特点,成为重点关注和重点管理防范的机构。本研究采用案例研究的方法,旨在对其面临的管理挑战进行深入剖析,为医养结合机构应对重大传染性疾病提供管理参考与借鉴。我们的研究发现,医养结合机构在重大传染性疾病暴发中面临着群体性感染风险高、对传染性疾病的预防和紧急治疗能力较弱、应急管理能力和应急物资储备不足、护理人力资源短缺导致的工作安排困难、服务质量下降、入住老人减少和运营成本上升等管理挑战。研究建议加强医养结合机构的医疗护理和预防融合能力、更新机构设施设计理念和标准使之更加符合防疫刚性要求和人性化居住需求、发挥智慧医疗和智慧照护优势,加快落实相关政策帮助该类机构渡过难关,实现可持续发展。
Integrated medical and elderly care institutions mostly accommodate senior citizens who are either semi-disabled or disabled,or demented,or chronically ill,thus becoming the focus of the management and prevention because of the high rate of severe illness and high mortality of the elderly people in the institutions when major infectious diseases break out.This case study aims to analyze the management challenges such institutions may face,and provide reference for the containing and controlling of major infectious diseases for them.The research has found that in the outbreak of major infectious diseases,integrated medical and elderly care institutions will be facing a higher risk of mass infection,weak prevention mechanism and emergency treatment of infectious diseases,insufficient emergency management capabilities or emergency material reserves,and shortage of nursing human resources.The resulting management challenges include difficulties in working arrangements,decreased service quality,fewer elderly residents,and increased operating costs.The research proposes to strengthen the medical care and prevention integration capabilities of these integrated institutions,update the design concepts and standards of institutional facilities,make them more in line with the rigid requirements of epidemic prevention and humanized housing needs,and give full play to the advantages of smart medical care.More emphasis is put on speeding up the implementation of related policies to help such institutions overcome difficulties and achieve sustainable development.
