高分辨率磁共振成像对女性压力性尿失禁盆底解剖结构评估 被引量:6

High Resolution MR Imaging in Assessment of Anatomic Abnormalities of Stress Urinary Incontinence in Women

摘要 目的利用高分辨率MRI评估女性压力性尿失禁(SUI)患者的尿道支持结构退变及损伤。方法采用Siemens Prisma 3.0 T超导磁共振成像仪,利用小视野ZOOMit T2WI三维薄层斜轴位图像,以不同年龄段的尿控正常者为对照组,分析SUI患者的尿道周围韧带、耻骨直肠肌及耻尾肌解剖结构变化。结果SUI患者40例,平均年龄(59.80±10.96)岁;对照1组30名,平均年龄(26.30±3.48)岁,对照2组30名,平均年龄(56.43±6.20)岁。较对照组,SUI组尿道周围韧带及耻尾肌退变损伤率增高,差异有统计学意义(P<0.01)。SUI组尿道周围韧带损伤率约80.0%,耻尾肌损伤率约72.5%。SUI组耻骨直肠肌肌束退变损伤率增高,但较对照2组,差异无显著统计学意义(P<0.05)。结论ZOOMit T2WI示SUI组尿道周围韧带及耻尾肌发生退变损伤率明显高于尿控正常组,耻骨直肠肌退变损伤常见于SUI组及已生育中老年尿控正常组。 Objective Using magnetic resonance imaging(MRI)to evaluate the morphology of periurethral supporting structures in female stress urinary incontinence(SUI).Methods All subjects have completed a small field of view(Zoomed imaging with parallel transmission technique,ZOOMit)T2-Weighted Imaging by Siemens Prisma 3 T MR.The anatomic morphology of pubococcygeal,puborectalis and periurethral ligament were assessed in SUI patients,compared with urinary controls.Results There were 40 SUI patients(mean age 59.80±10.96),30 young controls(mean age 26.30±3.48),and 30 older controls(mean age 56.43±6.20).Periurethral ligament defects were present in 80.0%,discontinuity or complete loss of pubococcygeal muscle was 72.5%in SUI,which was significantly different from the continent groups(P<0.01).Comparing to the older controls,there was no significant difference in the puborectalis defect in female stress urinary incontinence(P>0.05).Conclusion ZOOMit T2WI provides detailed morphologic information of the pelvic floor.SUI patients had a visible defect in the pubococcygeal muscle and periurethral ligament,which can be related to the weakness of urethral support.
