原发性胸腺神经内分泌肿瘤的临床特征及CT表现 被引量:6

摘要 目的探讨原发性胸腺神经内分泌肿瘤的临床及CT表现,提高对本病的认识。方法搜集经手术或穿刺病理证实的11例神经内分泌肿瘤患者,总结该肿瘤的临床特点及CT特征。结果病灶均位于前纵隔,出现囊变坏死6例。增强后不均匀强化8例;瘤内血管3例。心包受累6例,胸膜受累6例,周围血管侵犯7例,邻近肺组织侵犯或肺转移7例,淋巴结转移2例,骨转移3例,肝转移2例。术后或治疗后随访发现,4例出现复发和转移。结论原发性神经内分泌肿瘤是位于前纵隔一类少见类型肿瘤,具有体积大、多坏死少钙化、增强呈中高度不均匀强化等表现。CT能够了解肿瘤本身、周围组织受累及远处转移情况,对于评估手术风险及制定治疗方案起到一定辅助作用。 Objective To explore the clinical and CT manifestations of primary thymic neuroendocrine tumor and improve the understanding of the disease.Methods 11 cases of neuroendocrine tumor confirmed by operation or percutaneous biopsy pathology were collected to summarize the clinical and CT features of the tumor.Results All the lesions were located in the anterior mediastinum,and 6 cases had cystic necrosis.There were 8 cases with inhomogeneous enhancement and 3 cases with intratumoral vessels.Pericardial involvement was seen in 6 cases;pleural involvement in 6 cases;peripheral vascular involvement in 7 cases;adjacent lung tissue involvement or lung metastasis in 7 cases;lymph node metastasis in 2 cases;bone metastasis in 3 cases;liver metastasis in 2 cases.Tunor recurrence and metastasis was found in 4 cases.Conclusion Primary neuroendocrine tumor is a rare type of tumor located in the anterior mediastinum,which has the characteristics of being bulky,with significant necrosis and less calcification,and moderate and avid uneven enhancement.CT can provide display of tumor involvement of surrounding tissue and distant metastasis,and this plays a supplementary role in the assessment of surgical risk and the formulation of treatment plan.
