急性期轻度创伤性脑损伤患者的静息态自发脑活动及功能连接异常 被引量:10

Abnormal Resting Spontaneous Brain Activity and Functional Connectivity in Patients with Acute Mild Traumatic Brain Injury

摘要 目的采用静息态功能磁共振成像(rs-fMRI)技术,探讨轻度创伤性脑损伤(mTBI)患者的异常自发脑活动及功能连接(FC)改变,以及这种异常脑功能改变与创伤后认知障碍的相关性。方法搜集36例急性期mTBI患者和30名年龄、性别和受教育程度匹配的健康对照者进行了前瞻性研究。通过计算fMRI信号的低频振幅(ALFF)来测量自发性神经元活动,以ALFF发生改变的脑区作为种子点,进行基于种子点的静息态FC分析,偏相关分析检测fMRI信息与创伤后认知障碍之间的相关性。结果与健康对照组相比,急性期mTBI患者在左侧小脑后叶ALFF值明显降低,左侧辅助运动区ALFF值显著升高;创伤后认知功能评分与左侧辅助运动区的ALFF值升高呈负相关。左侧小脑前叶与左侧枕中回及左侧额中回之间的FC减少;左侧辅助运动区与左侧颞中回之间的FC减少。结论急性期mTBI患者在多发脑区存在ALFF及FC异常,并且部分脑区的ALFF值异常与创伤后认知障碍有关。这可能是mTBI后认知障碍的潜在神经病理生理机制之一。 Objective To investigate the abnormal spontaneous brain activity and functional connectivity in patients with mild traumatic brain injury(mTBI)and the correlation between such spontaneous abnormal brain function and posttraumatic cognitive impairment by using rest-state functional magnetic resonance imaging(rs-fMRI).Methods a prospective study of 36 acute mTBI patients and 30 healthy controls matched in age,sex,and education.The calculation of low frequency fluctuation of fMRI signal(amplitude of low frequency fluctuation,ALFF)was used to measure spontaneous neuronal activity,then seeing the brain areas with ALFF changes as a seed point,then resting state Functional connectivity based on seeds point(Functional connectivity,FC)analysis partial correlation analysis was used to detect fMRI information and the correlation between cognitive impairment after trauma.Results Compared with the healthy control group,the ALFF value in the left posterior cerebellar lobe of mTBI patients in the acute phase was significantly reduced,while the ALFF value in the left auxiliary motor area was significantly increased.There was a negative correlation between the scores of cognitive function and the increase of ALFF in the left assisted motor area.In addition,FC decreased between the left anterior cerebellar lobe and the left occipital gyrus and the left frontal gyrus.FC decreased between the left auxiliary motor area and the left temporal middle gyrus.Conclusion Patients with acute mTBI have abnormal ALFF and FC in multiple brain areas,and abnormal ALFF values in some brain areas are related to post-traumatic cognitive impairment.This may be one of the potential neuropathophysiological mechanisms of cognitive impairment after mTBI.
