Evaluation of Superomedial CNL Injury in 3.0T MRI

摘要 目的探讨弹簧韧带复合体内上束(smCNL)损伤的MRI表现。方法回顾性分析2015年2月至2017年2月行踝关节MRI检查的31例确诊为成人获得性扁平足(AAFD)的患者及20名正常受检者的影像学资料,分析两组受检者smCNL的MRI表现,并测量其近侧端和远侧端厚度。采用独立样本t检验比较两组间smCNL的近侧端、远侧端厚度。结果正常组MRI表现为smCNL呈T1WI等信号、T2WI-质子密度加权成像(PDWI)脂肪抑制序列低信号,结构清晰,内见疏松丝状结构,韧带紧贴距骨头表面,近端上方可见正常胫骨后肌腱,近侧端厚度(3.11±0.61)mm、远侧端厚度(0.91±0.20)mm。扁平足组MRI表现为smCNL结构不清,明显弥漫性增厚、边缘模糊、信号增高,近侧端厚度(4.09±1.36)mm、远侧端厚度(2.08±1.07)mm。两组受检者的smCNL近侧端、远侧端厚度差异均有统计学意义(t值分别为-3.03、-4.86,P均<0.05)。结论MRI诊断smCNL损伤确切,smCNL近侧端及远侧端厚度可为AAFD患者提供一定的诊断依据。 Objective To explore the accuracy diagnostic of the superomedial calcaneonavicular ligament(CNL)injury in patients with Adult-acquired flatfoot deformity by 3.0 T MRI.Methods Our retrospective review the ankle MRI data between identified The Adult-acquired flatfoot group comprising 31 cases of surgically or X-ray proved and 20-normal comparison group.Images were diagnosed by the radiologist for the abnormal signal features of superomedial CNL and measured the proximal and distal thickness of superomedial CNL.Results The normal ligaments have moderate signal intensity on T1-weighted and low on T2 PD-weighted MR images,showed a clearly loose filamentous structure,which closely running to the surface of the Talus bone and the normal posterior tibial tendon is visible above the proximal.The normal proximal and distal thickness of superomedial CNL was(3.11±0.61)mm and(0.91±0.20)mm,respectively.The superomedial CNL of The Adult-acquired flatfoot group were obviously diffusivity thickened,heighten on T2 PD-weighted MR images,the edge blurred.The proximal and distal thickness of superomedial CNL was(4.09±1.36)mm and(2.08±1.07)mm,significant different with the normal control group(t=-3.03;t=-4.86 both P<0.05).Conclusion 3.0 T MRI can acquire the accuracy diagnosis of superomedial CNL injury in The Adult-acquired flatfoot,the thickness of the superomedial CNL proximal and distal can provide basic information for diagnosis and treatment.
