摘要
目的探讨表观扩散系数(ADC)的全域直方图对子宫内膜癌与子宫黏膜下肌瘤的鉴别诊断价值。方法对经手术病理证实的79例子宫内膜癌和35例子宫黏膜下肌瘤患者的ADC图像进行回顾性分析,并用MaZda软件勾画感兴趣区(ROI)并进行直方图分析,对比两组ADC图像的灰度直方图参数,包括平均值(Mean)、方差(Variance)、偏度(Skewness)、峰度(Kurtosis)、第1百分位数(1th)、第10百分位数(10th)、第50百分位数(50th)、第90百分位数(90th)、第99百分位数(99th),绘制受试者工作特征曲线(ROC)分析以上参数评估子宫内膜癌与子宫黏膜下肌瘤的鉴别诊断效能。结果子宫内膜癌患者的ADC_(Mean)、ADC_(1th)、ADC_(10th)、ADC_(90th)和ADC_(99th)值均明显低于子宫黏膜肌瘤,ADC_(Variance)和ADC_(Kurtosis)值明显高于子宫黏膜下肌瘤(P<0.05),子宫内膜癌患者和子宫黏膜肌瘤患者间的ADC_(Skewness)值无明显统计学差异。其中,ADC10th值的曲线下面积最大(0.861),其敏感度和特异度分别为89.1%、78.9%。结论基于ADC图的全域直方图能够有效鉴别子宫内膜癌和子宫黏膜下肌瘤。
Objective The value of global histogram based on ADC chart in distinguishing endometrial carcinoma and submucosal fibroids.Methods The ADC images of 79 cases of endometrial cancer and 35 cases of submucosal fibroids confirmed by surgery and pathology were retrospectively analyzed,and the region of interest(ROI)was delineated by MaZda software and histogram analysis was performed to compare the ADC images of the two groups.Gray-level histogram parameters,including mean(Mean),variance(Variance),skewness(Skewness),peak value(Kurtosis),1 st percentile(1 th),10 th percentile(10 th),and 50 th percentile(50 th),90 th percentile(90 th),99 th percentile(99 th)were analyzed.We drew ROC curve to analyze the above parameters to evaluate the differential diagnosis efficacy of endometrial cancer and submucosal fibroids.Results The ADC_(Mean),ADC_(1 th),ADC_(10 th),ADC_(90 th)and ADC_(99 th)values of patients with endometrial cancer were significantly lower than those of uterine mucosal fibroids.The ADC_(Variance)and ADC_(Kurtosis)values were significantly higher than those of uterine submucosal fibroids(P<0.05).There was no significant difference in ADC_(Skewness)between patients with uterine fibroids.Among them,the area under the curve of ADC_(10 th)value is the largest(0.861),and its sensitivity and specificity are 89.1%and 78.9%,respectively.Conclusion The global histogram based on ADC chart can effectively distinguish endometrial cancer and submucosal fibroids.
