Apical root canal and cross-section anatomy morphology of mesial roots of mandibular first molar teeth by means of micro-computed tomography in Chinese population

摘要目的:使用显微CT研究下颌双根与三根第一恒磨牙近中根根尖根管与断面解剖形态。方法:对91颗三根型和150颗双根型下颌第一恒磨牙进行显微CT扫描,三维重建近中根根尖根管5 mm形态。研究根尖主根管的结构、侧副根管发生率、不同层面根尖切除去除根尖表面根尖孔的牙的比例、根尖峡区横断面和三维形态。结果:60.4%三根型为独立根管,41.3%双根型为融合根管;三根型和双根型侧副根管发生率为67.03%和66.67%;98.9%三根型在根尖切除4 mm和92.0%双根型在根尖切除3 mm去除根尖表面根尖孔。三根型峡区横断面Ⅰ类、三维形态Ⅳ类比双根型多;双根型峡区横断面Ⅴ类和单圆类、三维形态Ⅰ类比三根型多。结论:下颌第一恒磨牙在三根型和双根型存在一定的解剖差异,三根型和双根型在显微根尖手术需切除合适长度,以提高治疗效果。 Objective:To study the apical root canal morphology of mesial roots of three-rooted and two-rooted mandibular first molar teeth with micro-CT.Methods:Ninety-one three-rooted and 150 two-rooted mandibular first molar teeth were scanned with micro-CT and reconstructed the images of apical 5 mm root canals of mesial roots of specimens.The incidence of root canal structure type and accessory canal,the ratio of teeth with removal of apical foramens on apical surface through apical resection in different sections,the isthmus morphology of apical cross-section and mesiodistal view were studied.Results:Totally 60.4%of three-rooted has independent root canals,41.3%of two-rooted has fused root canal.The incidence of accessory canal in three-rooted and two-rooted are 67.03%and 66.67%,respectively.And 98.9%of three-rooted with removal of apical foramens on apical surface through apical 4 mm while 92.0%of two-rooted through apical 3 mm.Type I of cross-section and Type IV of three-dimensional view of isthmus in three-rooted is more than two-rooted.Type V and round canal of cross-section and Type I of three-dimensional view of isthmus in two-rooted is more than three-rooted.Conclusion:There are certain anatomical differences in between three-rooted and two-rooted mandibular first molars and apical resection of three-rooted and two-rooted should be appropriate length to improve treatment results.
