摘要
目的探讨经皮经椎间孔脊柱内镜技术治疗老年退变性腰椎管狭窄症的效果。方法选取2019年1月至2020年2月眉山市中医医院骨科收治的56例老年退变性腰椎管狭窄症患者作为研究对象,其中28例予传统椎板间开窗减压术治疗(对照组),余28例予经皮经椎间孔脊柱内镜技术治疗(观察组)。两组患者术后均随访6个月,比较其手术情况、临床疗效、腰椎功能和腰痛评分。计量资料的组间比较采用t检验,计数资料的组间比较采用χ^(2)检验。结果观察组患者手术时间和住院时间较对照组缩短,手术切口长度较对照组减小(t=6.176、8.885、18.670,P<0.05)。术后6个月时,对照组的总有效率为67.86%(19/28),观察组为85.71%(24/28),差异有统计学意义(χ^(2)=3.914,P<0.05)。与手术前比较,术后1 d及6个月时两组患者Oswestry功能障碍指数问卷表、疼痛视觉模拟评分均显著降低(P<0.05),且观察组低于对照组(P<0.05)。结论与传统椎板间开窗减压术相比,经皮经椎间孔脊柱内镜技术可以缩短手术和住院时间,进一步修复老年退变性腰椎管狭窄症患者的腰椎功能,减轻患者的腰痛程度,增强治疗效果。
Objective To evaluate the effect of percutaneous transforaminal spinal endoscopy in the treatment of senile degenerative lumbar spinal stenosis in elderly patients.Methods A total of 56 elderly patients with degenerative lumbar spinal stenosis admitted to Meishan Hospital of Traditional Chinese Medicine during January 2019 and February 2020 were enrolled in the study.28 patients treated with traditional interlaminar fenestration decompression(control group),and the other 28 patients were was treated with percutaneous transforaminal spinal endoscopy(observation group).Patients in both groups were followed up for 6 months.The operation situation,clinical efficacy,lumbar function and low back pain scores were compared between the two groups.T test and Chi-square test were used for comparing the difference of measurement data and enumeration data,respectively.Results Compared with the control group,the operation time and hospital stay in the observation group were shorter,and the length of surgical incision was shorter(t=6.176、8.885、18.670,P<0.05).The total effective rate of observation group(85.71%)was higher than that of control group(67.86%)(χ^(2)=3.914,P<0.05).Oswestry dability index and Visual analogue scale scores decreased in both groups 1 day and 6 months after operation(P<0.05),and the scores in the observation group were lower than those in the control group(P<0.05).Conclusion Compared with traditional interlaminar decompression,percutaneous transforaminal endoscopy spine technique can shorten operation time and hospital stay,restore the lumbar function and relieve pain in elderly patients with degenerative lumbar spinal stenosis.
