摘要
目的探讨快速康复外科(fast track surgery,FTS)理念在老年前列腺增生患者行经尿道等离子前列腺电切术围手术期中的应用效果。方法选取2019年3月至2020年9月在红河州第三人民医院行经尿道等离子前列腺电切术治疗的136例老年前列腺增生患者,其中68例患者围手术期进行常规护理(对照组),余68例患者围手术期在对照组的基础上进行FTS理念护理干预(观察组)。观察比较两组患者的术后康复情况和并发症发生情况。计量资料的组间比较采用t检验,计数资料的比较采用χ^(2)检验。结果观察组膀胱冲洗时间、导尿管留置时间、排气时间、住院时间均明显短于对照组,膀胱痉挛评分、生命质量评分、疼痛数字评分均明显低于对照组(t=41.078、10.295、19.852、44.851,P<0.05);观察组出血、尿路感染、膀胱痉挛、寒颤等并发症的发生率均明显低于对照组(χ^(2)=6.276、5.849、5.830、4.781,P<0.05)。结论FTS理念能够促进行经尿道等离子前列腺电切术的老年前列腺增生患者的术后康复,降低术后并发症的发生率。
Objective To explore the application effect of fast track surgery(FTS)concept in the perioperative period of transurethral plasma kinetic electrovaporization resection of prostate(TPKRP)in elderly patients with benign prostatic hyperplasia.Methods 136 elderly patients with prostatic hyperplasia treated with TUPKP admitted to the Third People’s Hospital of Honghe Prefecture from March 2019 to September 2020 were selected.Among them,68 patients received routine nursing during perioperative period(control group),and the other 68 patients received FTS concept nursing intervention on the basis of the control group(observation group).The postoperative rehabilitation and complications of the two groups were observed and compared.T test andχ^(2)test were used for the comparison of measurement data and counting data between groups,respectively.Results The bladder flushing time,catheter retention time,exhaust time and hospital stay in the observation group were significantly shorter than those in the control group(t=41.078,10.295,19.852,44.851,P<0.05),and the bladder spasm score,quality of life score and pain number score in the observation group were significantly lower than those in the control group(t=14.005,3.444,7.080,P<0.05).The incidence of complications such as bleeding,urinary tract infection,bladder spasm and shivering in the observation group was significantly lower than that in the control group(χ^(2)=6.276,5.849,5.830,4.781,P<0.05).Conclusion The FTS concept can promote the postoperative rehabilitation of elderly patients with prostatic hyperplasia undergoing TUPKP and reduce the incidence of postoperative complications.
