摘要
目的 观察健脾益肺汤结合桉柠蒎肠溶软胶囊治疗支气管扩张症(稳定期)的临床疗效及对血清炎症水平的影响。方法 以2019年10月—2020年12月该院收治的支气管扩张症(稳定期)患者73例为研究对象,依据治疗方法的不同分为观察组(38例)和对照组(35例),对照组采取常规抗感染、抗炎、扩张气道等西医常规治疗,口服桉柠蒎肠溶软胶囊,0.3 g/次,3次/d。观察组桉柠蒎肠溶软胶囊联合健脾益肺汤治疗。两组均按疗程治疗28 d。比较两组治疗前后中医证候积分变化和治疗疗效,比较两组治疗前后血清炎症因子白细胞(WBC)、中性粒细胞(GRAN)、C反应蛋白(CRP)水平变化,比较两组肺功能第1秒用力呼吸容积(FEV1)、用力肺活量(FVC)和肺一氧化碳弥散量(DLCO)水平变化。结果 对照组治疗总有效率低于观察组(P<0.05);观察组治疗后中医证候积分降低幅度优对照组(P<0.05);观察组治疗后功能指标FEV1、FVC和DLCO高于对照组(P<0.05);观察组治疗后血清炎症因子白细胞(WBC)、中性粒细胞(GRAN)和C反应蛋白(CRP)水平低于对照组(P<0.05)。结论 健脾益肺汤结合桉柠蒎肠溶软胶囊治疗支气管扩张症(稳定期)疗效确切,可有效的改善患者的临床症状,降低炎症反应和病情的严重性,改善肺功能和气流受阻程度,提高治疗疗效,值得临床借鉴。
Objective To observe the clinical effect of Jianpi Yifei Decoction(健脾益肺汤)combined with Eucalyptol,Limonene and Pinene Enteric Soft Capsules in the treatment of bronchiectasis(stable phase)and its effect on serum inflammation.Methods Seventy-three patients with bronchiectasis(stable phase)admitted to the hospital from October 2019 to December 2020 were selected as the research objects.Based on the different treatments,the patients were divided into the observation group(38 cases)and control group(35 cases).The control group was treated with conventional Western medicine treatment,such as anti-infection,anti-inflammatory and airway dilation,and took Eucalyptol,Limonene and Pinene Enteric Soft Capsules orally(0.3 g/time,3 times/d).On the basis of the treatment in the control group,the observation group took Jianpi Yifei Decoction orally.Both groups were treated according to the course of treatment for 4 weeks.The changes of TCM syndrome scores and therapeutic efficacy were compared between the two groups before and after treatment.The changes of serum inflammatory factors[white blood cells(WBC),neutrophil count(GRAN),C-reactive protein(CRP)]and lung function[forced expiratory volume in the first second(FEV1),forced vital capacity(FVC)and diffusion capacity for carbon monoxide(DLCO)]were compared between the two groups.Results The total effective rate of the observation group(89.47%)was higher than that of control group(68.57%)(P<0.05).The scores of cough,sputum,chest tightness,shortness of breath,fatigue,lethargy and loose stool in the observation group were lower than those in the control group(P<0.05).After treatment,the functional indexes(FEV1,FVC and DLCO)in the observation group were higher than those in the control group(P<0.05).The levels of serum inflammatory cytokines(WBC,GRAN and CRP)in the observation group were lower than those in the control group after treatment(P<0.05).Conclusion Jianpi Yifei Decoction combined with Eucalyptol,Limonene and Pinene Enteric Soft Capsules in the treatment of bronchiectasis(stable stage)has exact effect and can effectively relieve the patients′clinical symptoms,reduce the inflammatory response and the severity of the disease,improve the lung function,the degree of airflow obstruction and the curative effect.It is worthy of clinical reference.
