摘要
目的探讨丹芪祛瘀止痛颗粒结合三联疗法对气虚血瘀型慢性萎缩性胃炎(chronic atrophic gastritis,CAG)患者氧化-抗氧化系统及血清骨桥蛋白(OPN)、胃饥饿素(Gherlin)的影响。方法选择医院于2018年8月—2019年11月诊断为幽门螺杆菌(Hp)阳性慢性萎缩性胃炎(CAG)患者146例,按照随机表法分为两组,每组73例,对照组给予三联联抗Hp治疗,研究组在对照组基础上加用丹芪祛瘀止痛颗粒治疗,比较两组患者临床疗效、Hp阴转率、胃镜改善情况、氧化-抗氧化系统指标改善情况、血清OPN、Gherlin水平变化。结果研究组治疗有效率明显高于对照组,差异有统计学意义(P<0.05)。研究组Hp转阴率较对照组明显升高(P<0.05)。两组患者治疗后临床症状、体征及胃镜显示均较治疗前好转,研究组较对照组好转明显(P<0.05)。两组患者治疗后血清SOD、CAT水平较治疗前升高,研究组升高更明显,血清MDA水平较治疗前下降,研究组下降更明显(P<0.05)。治疗后两组患者OPN水平均较治疗前下降,且研究组下降更明显,两组患者Ghrelin水平均较治疗前上升,且研究组上升更明显(P<0.05)。对照组不良反应发生率为8%(61/73),研究组不良反应发生率为12%(9/73),差异无统计学意义(P>0.05)。结论丹芪祛瘀止痛颗粒结合三联疗法能够改善气虚血瘀型CAG患者症状,提高临床疗效,减轻胃黏膜氧化损伤,降低血清OPN水平,升高体内Ghrelin水平,促进疾病恢复。
Objective To explore the curative efficacy of Danqi Quyuzhitong Granules(丹芪祛瘀止痛颗粒)combined with triple therapy in the treatment of chronic atrophic gastritis(CAG)due to deficiency of Qi and blood stasis and its effects on oxidative-antioxidant system,osteopontin(OPN)and Gherlin.Methods A total of 146 patients diagnosed as Helicobacter pylori(Hp)CAG in the hospital from August 2018 to November 2019 were selected and divided into two groups according to the random table method,with 73 cases in each group.The control group was given triple anti-Hp treatment,and the study group was treated with Danqi Quyu Zhitong Granules on the basis of the control group.The clinical efficacy,Hp negative conversion rate,gastroscopy improvement and oxidation-antioxidation were compared between the two groups.The improvement of oxidative system indicators,and changes in serum levels of OPN and Gherlin of two groups were observed.Results The effective rate of treatment in the study group was significantly higher than that in the control group,and the difference was statistically significant(P<0.05).The negative rate of H.pylori in the study group was significantly higher than that of the control group(P<0.05).After treatment,the clinical symptoms,signs and gastroscopic results of the two groups were better than those before treatment,and those of the study group were significantly better than those of the control group(P<0.05).After treatment,the serum levels of superoxide dismutase(SOD)and catalase(CAT)of the two groups were increased compared with those before treatment,the increase was more obvious in the study group.The serum malondialdehyde(MDA)level was decreased compared with that before treatment,and the decrease in the study group was more obvious(P<0.05).After treatment,the OPN levels of the two groups were decreased compared with those before treatment,and the decrease was more obvious in the study group.After treatment,the Ghrelin levels of the two groups were increased compared with those before treatment.And the increase in the study group was more obvious(P<0.05).The incidence of adverse reactions in the control group was 8%(6/73)and that in the study group was 12%(9/73),and the difference was not statistically significant(P>0.05).Conclusion Danqi Quyuzhitong Granules combined with triple therapy can relieve the symptoms of CAG patients with Qi deficiency and blood stasis,improve the clinical efficacy,reduce the gastric mucosal oxidative damage and serum OPN levels,increase Ghrelin levels and promote the disease recovery.
