摘要
目的:探讨参麦注射液对无症状性心肌缺血型冠心病患者的临床疗效及其对患者心电图的影响。方法:选取90例无症状性心肌缺血型冠心病患者,随机分为对照组及观察组,每组45例。对照组给予常用抗血小板聚集、降脂、营养心肌等常规西医治疗。观察组在对照组的基础上联合参麦注射液治疗,14 d为1个周期,连续治疗2个周期。统计两组患者治疗前后血流动力学、窦性心率震荡(HRT)及心率变异性(HRV)时域分析指标以及心电图的变化情况。结果:观察组总有效率为91.8%,显著高于对照组的77.6%(P<0.01);与治疗前比较,治疗后观察组患者EF、CO及SV均明显升高,且显著高于对照组(P<0.01);而对照组EF、CO及SV均无显著变化(P>0.05);与治疗前比较,治疗后两组患者TO显著降低,而TS显著升高,两组间差异显著(P<0.01);与治疗前比较,治疗后两组SDNN、SDANN、RMSSD及PNN50均显著升高(P<0.01),且治疗组显著高于对照组(P<0.01);治疗后两组患者V5导联ST段下移、V5导联T波改变及Ptf V1负值增大现象均有所改善,且观察组改善率均显著高于对照组(P<0.05或P<0.01)。结论:参麦注射液可显著改善无症状性心肌缺血型冠心病患者的血流动力学、HRT及HRV异常状态,且心电图改善率及临床疗效均显著高于常规西药治疗。
Objective:The clinical effect of Shenmai injection on patients with asymptomatic myocardial ischemic coronary heart disease and its effect on electrocardiogram would be investigated.Methods:Ninety patients with asymptomatic myocardial ischemic coronary heart disease will be choosed,45 cases each,were randomly divided into control group and observation group.The control group was treated with routine western medicine such as anti-platelet aggregation,lipid-lowering and myocardial nutrition,while the observation group was treated with Shenmai injection on the basis of the control group.The treatment lasted for 14 days for one cycle and lasted for 2 cycles.The changes of hemodynamics,HRT,HRV and electrocardiogram before and after treatment were analyzed.Results:The total effective rate of the observation group was 91.8%,which was significantly higher than 77.6%of the control group(P<0.01);EF,CO and SV in the observation group were significantly higher than those in the control group after treatment(P<0.01);EF,CO and SV in the control group had no significant changes(P>0.05);Compared with before treatment,TO in the two groups decreased significantly after treatment,while TS increased significantly,and the difference between the two groups was significant(P<0.01).Compared with before treatment,SDANN,RMSSD and PNN50 in the two groups increased significantly after treatment(P<0.01),and the treatment group was significantly higher than that in the control group(P<0.01).After treatment,ST segment of V5 lead,T wave change of V5 lead and negative increase of Ptf V1 in both groups were improved,and the improvement rate in the observation group was significantly higher than that in the control group(P<0.05 or P<0.01).Conclusion:Shenmai injection can significantly improve the abnormal state of hemodynamics,HRT and HRV in patients with asymptomatic myocardial ischemic coronary heart disease,and the improvement rate of electrocardiogram and clinical efficacy are significantly higher than those of conventional western medicine.
