摘要
目的探讨早产儿血清NSE水平升高的危险因素。方法回顾性分析入住新生儿病房150例早产儿的临床资料。以是否发生NSE升高(>36.89μg/L),将早产儿分为血清NSE升高组(58例)和血清非NSE升高组(92例)。对可能导致NSE升高的影响因素进行单因素分析和非条件多因素logistic回归分析。结果单因素分析显示,两组间在脐带异常、母孕期糖尿病、出生体重、新生儿高胆红素血症、新生儿低血糖、新生儿窒息、新生儿败血症、顺产等方面比较,差异有显著性(P<0.05)。非条件多因素logistic回归分析显示,低出生体重(OR=1.002,95%CI:1.00-1.00)、新生儿低血糖(OR=2.349,95%CI:0.75-7.34)、新生儿窒息(OR=0.183,95%CI:0.06-0.54)是早产儿血清NSE升高的独立危险因素。结论低出生体重儿、新生儿窒息、新生儿低血糖可导致NSE升高。
Objective To study the risk factors for neuron-specific enolase in preterm infants.Methods A retrospective analysis was performed for the clinical data of 150 preterm infants who were admitted to the Neonatal Intensive Care Unit(NICU).According to the presence or absence of elevated NSE(NSE>36.89μg/L),the preterm infants were divided into elevated NSE group with 58 infants and nonelevated NSE group with 92 infants.Univariate analysis and unconditional multivariate logistic regression analysis were performed on the influencing factors that may cause NSE to increase.Results The univariate analysis showed that there were significant differences between the elevated NSE group and the non-elevated NSE group in umbilical cord abnormality,maternal diabetes mellitus,birth weight,hyperbilirubinemia of newborn,neonatalhypoglycemia,neonatal asphyxia,septicemia of newborn,eutocia(P<0.05).The unconditional multivariate logistic regression analysis showed that low birth weight(OR=1.002,95%CI:1.00-1.00),neonatal hypoglycemia(OR=2.349,95%CI:0.75-7.34)and neonatal asphyxia(OR=0.183,95%CI:0.06-0.54)were independent risk factors for elevated NSE in preterm infants.Conclusion Low birth weight,neonatal asphyxia and neonatal hypoglycemia lead to elevated NSE in preterm infants.
