摘要
目的系统评价γ干扰素释放试验、核酸检测试验、结核抗体检测试验3种方法用于泌尿生殖系统结核的诊断价值。方法检索PubMed、Embase、Web of science、Clarivate、知网、万方、维普电子数据库从建库到2023年1月关于诊断泌尿生殖系统结核方法的相关文献,提取文献数据,采用Stata 16、Meta-Disc 1.4和RevMan 5.4软件进行数据分析和质量评估。结果最终纳入的32篇相关文献的分析结果显示:对于合并敏感度、合并特异度、合并阳性似然比、合并阴性似然比、合并诊断比值比、综合受试者操作特征曲线下面积,γ干扰素释放试验分别为0.84,0.77,3.58,0.23,17.95,0.89;核酸检测试验分别为0.65,0.94,7.57,0.29,38.50,0.92;结核抗体检测试验分别为0.66,0.90,5.54,0.37,17.68,0.82。根据sROC曲线下面积排序、Fagan列线图、似然比图和概率修正图,核酸检测试验的诊断准确性和临床诊断价值最高。同时使用回归分析了3种方法异质性的来源。结论核酸检测试验对于泌尿生殖系统结核具有更高的诊断价值,可以用于提高临床对泌尿生殖系统结核的诊断准确性。
Objective To systematically evaluate the diagnostic value of three methods Interferon-gamma release assay,nucleic acid detection and tuberculosis antibody detection for urogenital tuberculosis.Methods The relevant articles was retrieved in PubMed,Embase,Web of science,Clarivate,CNKI,Wanfang,Vip electronic databases on the diagnosis of urogenital tuberculosis,from the establishment of the database to January 2023.The literature data were extracted.The Stata 16,Meta-Disc 1.4 and RevMan 5.4 softwares were used for data analysis and quality evaluation.Results The analysis results of 32 related articles finally included showed that:for the combined sensitivity,combined specificity,combined positive likelihood ratio,combined negative likelihood ratio,combined diagnostic odds ratio,and the area under the summary receiver operating characteristic curves,the Interferon-gamma release assay was 0.84,0.77,3.58,0.23,17.95 and 0.89;The nucleic acid detection test was 0.65,0.94,7.57,0.29,38.50 and 0.92;The tuberculosis antibody test was 0.66,0.90,5.54,0.37,17.68 and 0.82,respectively.According to the area under summary receiver operating characteristic curves,Fagan plots,likelihood ratio scattergram and probability modifying plot,the nucleic acid detection test has the highest diagnostic accuracy and clinical diagnostic value.At the same time,regression analysis was used to analyze the sources of heterogeneity of the three methods.Conclusion Nucleic acid detection has the higher diagnostic value for urogenital tuberculosis,which can improve the clinical diagnostic accuracy of urogenital tuberculosis.
