立体定向脑电图检测下射频热凝毁损术对癫痫患者MoCA评分、疾病发作频率及不良反应的影响 被引量:6

摘要 目的探究立体定向脑电图检测下射频热凝毁损术对癫痫患者MoCA评分、疾病发作频率及不良反应的影响。方法研究对象为2015年1月至2018年1月收治的38例癫痫患者,所有患者采用立体定向脑电图检测下射频热凝毁损术进行治疗,回顾性分析患者临床资料。结果38例患者均成功植入立体脑电图电极明确致痫灶,且无脑出血、颅内感染等并发症。患者术后MoCA评分为(28.16±3.52)分,均未出现神经认知功能障碍;Engel分级情况:I级30例,Ⅱ级5例,Ⅲ级2例,Ⅳ级1例,其中3例患者热凝后无发作;热凝过程中4例出现先兆,3例出现头痛、恶心,立即停止热凝后症状均消失;术后有6例患者出现不同程度的高热、嗜睡和记忆力下降等不良反应,对症治疗后均恢复。结论立体定向脑电图检测下射频热凝毁损术是一种微创、精准性的治疗方法,对神经认知功能损伤小,治疗后不良反应少,安全度高,可有效治疗难治性颞叶癫痫。 Objective To investigate the effects of radiofrequency thermocoagulation damage under stereotactic electroencephalogram(SEEG)detection on MoCA score,disease onset frequency and adverse reactions in epilepsy patients.Methods The study objects were the 38epilepsy patients who were admitted from January 2015to January 2018.All patients were given treatment with radiofrequency thermocoagulation damage under SEEG detection.The clinical data of patients were retrospectively analyzed.Results All 38patients were successfully implanted with stereo EEG electrodes to identify epileptogenic foci.There were no complications such as cerebral hemorrhage and intracranial infection.The postoperative MoCA score was(28.16±3.52)points.There was no neurocognitive dysfunction.Engel classification:30cases at grade I,5 cases at grade II,2cases at grade III,1case at grade IV.There were 3cases without onset after thermocoagulation.4cases with aura in thermocoagulationprocess.There were 3cases with headache and nausea,symptoms were disappeared immediately after stopping thermocoagulation.There were 6cases with adverse reactions after surgery such as different degrees of high fever,lethargy and memory loss.They were recovered after symptomatic treatment.Conclusion Radiofrequency thermocoagulation damage under SEEG detection is a minimally invasive and accurate treatment method.There is little damage to neurocognitive function,and few adverse reactions after treatment,with high safety,which can effectively treat refractory temporal lobe epilepsy.
