摘要
吞咽障碍是脑卒中患者最常见的功能障碍之一,会引发吸入性肺炎、营养不良等诸多并发症,针刺是治疗卒中后吞咽障碍的有效方法。本文对近十年针刺治疗中风后吞咽障碍的临床研究文献进行整理总结,发现:腧穴选择以舌咽与颈项局部为主,同时根据整体辨证有选择地结合肢体部、头面部、躯干部、头皮部腧穴;在针刺方案的选择上以醒脑开窍针刺法为代表的具有明确组穴方案及命名的针法占比最多,其次为以针刺区域命名的针法、特殊针具针刺方法;而对照组的选择上,以吞咽康复训练为最多,也有以脑血管病常规药物治疗、低频电刺激为对照。观察指标的选择上,根据评估方法与目的,主要包括仪器评估、量表评估、综合状态评估、机制探讨等。在针刺介入时机上,介入时间越早临床疗效越佳,而且针刺对咽期吞咽障碍的效果更佳。
Dysphagia is one of the most common functional disorders of cerebral apoplexy,which can lead to inhalation pneumonia,malnutrition and many other complications.Acupuncture is an effective method to treat dysphagia after stroke.In this paper,the clinical research literature on acupuncture for the treatment of dysphagia after apoplexy in recent 10 years was summarized.The selection of acu-points is mainly based on the tongue pharynx and neck,and selectively combine the acupoints of limbs,head and face,trunk and scalp according to the overall dialectics;In the selection of acupuncture programs,the acupuncture method represented by the xingnao Kaiqiao acupuncture method with definite acupoint plan and named accounted for the most,followed by the acupuncture area named,followed by the special needle needling method.The choice of the control group,swallowing rehabilitation training as the most,there are also to cerebrovascular disease routine drug treatment,low frequency electrical stimulation as the control.According to the evaluation method and purpose,the selection of observation index mainly includes instrument evaluation,scale evaluation,comprehensive state evaluation,mecha-nism discussion and so on.In terms of the timing of acupuncture intervention,the earlier the intervention time,the better the clinical effect,and the better the effect of acupuncture on swallowing disorders in pharyngeal stage.
