摘要
目的:探讨显微镜下Mathieu术式治疗阴茎型尿道下裂的临床疗效。方法:回顾性分析2016年1月-2019年6月期间收治53例阴茎型尿道下裂患儿资料,根据是否在显微镜下手术分为研究组33例和对照组20例。研究组采用ZEISS 88显微镜下Mathieu术尿道成形术,对照组采用传统Mathieu术尿道成形术,比较两组手术疗效及并发症。结果:研究组患儿尿道重建长度(2.9±1.1)cm,尿道板宽度(1.4±0.5)cm;术后出现尿瘘1例,行显微镜下尿瘘修补治愈,尿道外口裂开移位至冠状沟处1例,术后无狭窄、阴茎伸直良好,并发症发生率6.1%。对照组患儿尿道重建长度(3.1±1.2)cm,尿道板宽度(1.5±0.4)cm;术后出现尿瘘2例,后行显微镜下修补,术后狭窄2例,行尿道扩张后排尿改善,术后所有患儿阴茎伸直良好,并发症发生率20%。结论:显微镜下Mathieu术式较传统术式治疗尿道下裂手术成功率高,并发症少,值得临床推广。
Objective:To investigate the clinical efficacy of the Mathieu procedure under microscope in the treatment of penile hypospadias.Methods:A retrospective analysis of 53 patients with penile hypospadias in our urology department from January 2016 to June 2019 was done.According to whether the procedure was operated under the microscope,the patients were divided into the study group(n=33)and the control group(n=20).The study group was given the Mathieu urethroplasty under the ZEISS 88 microscope,and the control group traditional Mathieu urethroplasty.The efficacy and complications of the two groups were compared.Results:The length of urethral reconstruction in the study group was(2.9±1.1)cm,and the width of urethral plate was(1.4±0.5)cm.One case of urinary fistula occurred after operation,and the urethral fistula was repaired under the microscopy.One case had a dehiscence of the external urethra and shifted to the coronal sulcus.There was no stenosis,the penis straightened well,and the complication rate was 6.1%.In the control group,the length of urethral reconstruction was(3.1±1.2)cm,and the width of urethral plate was(1.5±0.4)cm.Two cases of urinary fistula occurred after surgery,which was repaired under microscope,and two cases of postoperative stenosis were cured after urethral dilation.The penis straightened well in all patients after the operation,and the overall complication rate was 20%.Conclusion:The Mathieu proceure under the microscopy has a higher success rate and fewer complications than the traditional operation in the treatment of hypospadias,which is worthy of clinical promotion.
