摘要
目的:对比硕通镜与输尿管镜治疗单侧输尿管结石的临床疗效及安全性。方法:回顾性分析恩施州中心医院2019年2月-2020年7月收治的118例单侧输尿管结石患者的临床资料,其中硕通镜组58例,输尿管镜组60例。比较两组患者的手术时间、术后住院天数、治疗费用、术后1个月结石清除率、Ⅰ期进镜失败率、Ⅱ期碎石发生率、术后发热发生率、输尿管损伤发生率,并按结石部位不同,分为输尿管上段和输尿管中下段进行亚组分析。结果:两组的手术时间、术后1个月结石清除率差异有统计学意义(P<0.05);在术后住院天数、治疗费用、Ⅰ期进镜失败率、Ⅱ期碎石发生率、术后发热发生率、输尿管损伤发生率比较差异无统计学意义(P>0.05)。对于输尿管上段结石,两组的手术时间、术后1个月结石清除率差异有统计学意义(P<0.05);两组的术后发热发生率、输尿管损伤发生率差异无统计学意义(P>0.05)。对于输尿管中下段结石,两组在手术时间比较差异具有统计学意义(P<0.05);两组在术后1个月结石清除率、术后发热发生率、输尿管损伤发生率比较差异无统计学意义(P>0.05)。结论:硕通镜与输尿管镜治疗单侧输尿管结石均安全有效。与输尿管镜相比,硕通镜在治疗输尿管上段结石时手术时间更短、结石清除率更高。
Objective:To compare the clinical efficacy and safety of Sotn ureterorenoscope and ureteroscopy in the treatment of unilateral ureteral calculi.Methods:The clinical data of 118 patients with unilateral ureteral calculi admitted to Enshi Central Hospital from February 2019 to July 2020 were retrospectively analyzed,including 58 cases in the Sotn ureterorenoscope group and 60 cases in the ureteroscopy group.Compare two groups of operation time,postoperative hospitalization days and cost of treatment,stone removal rate of 1 month after operation,a period into the mirror failure rate,Incidence of stageⅡlithotripsy,the incidence of postoperative fever,ureteral injury incidence rate.Patients were divided into upper ureteral segment and middle and lower ureteral segment for subgroup analysis according to different calculi locations.Results:There were statistically significant differences between the two groups in terms of operation time,stone removal rate of 1 month after operation(P<0.05).There were no statistically significant differences between the two groups in postoperative hospital days,treatment costs,a period into the mirror failure rate,Incidence of stageⅡlithotripsy,the incidence of postoperative fever,ureteral injury incidence rate(P>0.05).For upper ureteral calculi,there were statistically significant differences between the two groups in terms of operation time,stone removal rate of 1 month after operation(P<0.05).There was no significant difference in the incidence of fever and ureteral injury between the two groups(P>0.05).For middle and lower ureteral calculi,the difference in operation time between the two groups was statistically significant(P<0.05).There were no statistically significant differences between the two groups in stone removal rate of 1 month after operation,incidence of postoperative fever and incidence of ureteral injury(P>0.05).Conclusion:The treatment of unilateral ureteral calculi by both Sotn ureterorenoscope and ureteroscopy is safe and effective.Compared with ureteroscopy,Sotn ureterorenoscope had shorter operative time and higher SFR in the treatment of upper ureteral calculi.
