摘要
目的:比较分析经尿道前列腺电切除术(TURP)和经尿道铥激光前列腺剜除术(ThuLEP)两种术式术后早期尿控情况。方法:采用回顾性研究,入组2021年1月至12月北京大学第一医院泌尿外科收治的前列腺增生患者44例,根据手术方式分为TURP组15例和ThuLEP组29例。比较两组患者围手术期指标和尿控情况。结果:TURP组和ThuLEP组排尿和尿控状况(IPSS,QOL和ICIQ-MLUT)与术前比较均有明显改善,差异有统计学意义(P<0.05)。与ThuLEP组比较,TURP组患者术后1个月时尿控情况优于ThuLEP组,差异有统计学意义(Z=-2.255,P=0.024),但术后6个月时,两组之间尿控情况不存在统计学差异。结论:TURP和ThuLEP的疗效和安全性相当。ThuLEP早期尿失禁发生概率高于TURP,但术后6个月时两组患者均没有尿失禁发生。
Objective:To compare the early urinary control of thulium laser enucleation of prostate(ThuLEP)vs.transurethral resection of prostate(TURP).Methods:A retrospective study was conducted on 44 patients with benign prostatic hyperplasia(BPH)admitted to the Department of Urology,Peking University First Hospital from January to December 2021.The patients were divided into TURP group(n=15)and ThuLEP group(n=29).Perioperative indicators and urinary control indicators were compared between the two groups.Results:Urination and urinary control(IPSS,QOLand ICIQ-MLUT)were significantly improved in TURP and ThuLEP groups after surgery,with statistically significant difference(P<0.05).The improvement of urinary continence in TURP group was better than that in ThuLEP group,and the difference was statistically significant(Z=-2.255,P=0.024)at 1st month after surgery,but no significant difference was seen at 6 month after surgery.Conclusion:TURP and ThuLEP have similar efficacy and safety.The incidence of early urinary incontinence in ThuLEP was higher than that in TURP.At 6th month postoperatively,no urinary incontinence occurred in either group.
