摘要
目的:国内具有完全自主知识产权的“多模态影像引导前列腺穿刺手术导航系统”成功完成了多例临床手术,该模型检证了系统的安全性和有效性。方法:经过多次测试,配制了与人体的声学和核磁技术参数相符的模型材料,设计制作了经直肠超声成像的人体和病灶模型,用超声仪测量了硅胶和琼脂人体模型影像的尺寸、相对位置、声强幅度。结果:在超声横向方向上硅胶靶球和琼脂模型的长度误差≤±1mm。在超声深度方向上,琼脂模型长度误差≤±1mm,硅胶靶球直径误差约为+12%。测量了模型的声速、声衰减系数、杨氏模量和泊松比,均与人体组织接近。体模和病灶的超声和核磁影像清晰准确。在体模不同位置和方向上设置了模拟直肠,从8个方向保证靶球的完整成像,使穿刺次数增加了8倍。结论:此款体模满足了模型的适度仿真、MRI-TRUS成像的准确性和实用性要求,也可用于浅表甲状腺和胸腺的超声影像和穿刺手术训练,是检验MRI-TRUS融合影像引导手术、化疗粒子植入、穿刺活检的理想体模。
Objective:The first fusion-image-guided navigation system for prostate needle biopsy with completely independent intellectual property rights in China has successfully been applied to 15 clinical operations.A tissue mimic(TM)is urgently proposed to cooperate with the tests for the safety and effectiveness of the system.Methods:Our ultrasound prostate training phantom is a multi-modality disposable phantom developed for testing and practicing procedures which involve scanning the lesions with 3 rectal probes.The acoustic and MRI parameters of the TM are consistent with those of human tissue.The geometry,relative positions,and intensity of the ultrasound imaging of the gel and agar formulations have been measured with an SIUI Apogee 2300 ultrasound instrument.Results:The error of measured lengths on US imaging of the gel lesions and the agar TM in the transverse direction of ultrasound is less than±1mm by comparing with the measured lengths of real objects with a micrometer and magnetic resonance imaging.In the direction of ultrasonic depth,the error of measured length of agar TM is less than±1mm.The diameters of gel lesions are larger with an error of+12%.We design experiments to measure the sound speeds and attenuation coefficients of the TMs,which are close to those of human tissue.The ultrasound and magnetic resonance imaging of lesions and tissue phantom are clear and accurate.The phantom is a consumable because damage is irreparable after puncture practice.To increase usage of phantom,we set simulated rectums in 3 positions and directions.The design ensures complete imaging of the lesions in 8 directions.It is economic and efficient.Conclusions:This phantom meets the requirements of high simulation,high accuracy and practicability of MRI-TRUS imaging of the phantom.The phantom can also be used for ultrasound imaging and needle biopsy training to superficial thyroid and thymus.This phantom is an ideal training device for ultrasound guided cryosurgery,radioactive seed implantation,and needle biopsy.
