靶向前列腺特异性膜抗原的放射引导手术在前列腺癌治疗中的应用进展

摘要 前列腺癌是男性泌尿系统常见肿瘤,前列腺癌根治手术的技术更新是前列腺癌治疗领域中备受关注的热点。靶向前列腺膜特异性膜抗原的放射引导手术(PSMA-RGS)可在术中提高对于微小转移淋巴结的识别与清除能力,有助于改善初诊以及复发前列腺癌患者的治疗预后。当前,多种放射性标记的小分子抑制剂已被应用于PSMA-RGS中,而最近出现的插入式(DROP-IN)探头则推动了放射引导手术与微创腹腔镜下机器人辅助手术环境的整合。本文从放射标记靶向配体优化、手术技术的进步以及临床应用现状三个方面综述近年来PSMA-RGS用于前列腺癌治疗的研究进展。 Prostate cancer is a common tumor in the male urinary system.The technical improvements of radical prostatectomy is a hot topic in the field of prostate cancer treatment.Radioguided surgery targeting prostate-specific membrane antigen(PSMA-RGS)has enhanced the efficiency to detect and remove micrometastatic lymph nodes dur⁃ing the radical prostatectomy,potentially improving the prognosis for both primary and recurrent prostate cancer.Cur⁃rently,various radiolabeled small molecule inhibitors have been appplied in PSMA-RGS,while the recent emergence of the DROP-IN probe has further facilitated the integration of RGS and minimally invasive robot-assisted laparoscop⁃ic surgery.This paper reviews the progresson PSMA-RGS for prostate cancer treatment,focusing on the optimization of radiolabeled targeted ligands,evolution of surgical techniques,as well as the relevant clinical applications.
