摘要
目的:评价机器人辅助腹腔镜技术在根治性膀胱切除术盆腔淋巴结清扫过程中的可行性和潜在优势。方法:回顾性分析2016年1月至2019年10月期间,在我院接受机器人辅助腹腔镜根治性膀胱切除术(RARC)患者62例,腹腔镜根治性膀胱切除术(LRC)患者72例。对比两组患者在肿瘤临床分期、病理分级、手术时间、术中出血量、术后住院时间、淋巴结清扫数量、清扫淋巴结的阳性率和围手术期并发症等指标之间的差异。结果:RARC组相比LRC组患者术中出血量少[(210.0±170.0)ml vs.(515.7±401.5)ml,P<0.001]、手术时间短[(344.8±121.2)min vs.(466.2±119.5)min,P<0.001]、术中清扫的淋巴结数量多[(18.2±8.7)个vs.(12.3±6.2)个,P<0.001]。两组患者术后住院时间、淋巴结阳性率和围手术期并发症方面差异无统计学意义。结论:相比传统腹腔镜技术,机器人辅助腹腔镜手术清扫的淋巴结数量多、手术时间短、术中出血量少,具有一定的优势。
Objective:To evaluate the potential advantage of robotic-assisted radical cystectomy with bilateral pelvic lymph node dissection compared with conventional laparoscopic radical cystectomy.Methods:From January 2016 to October 2019,62 patients were performed by robotic-assisted radical cystectomy(RARC),and 72 patients were performed by laparoscopic radical cystectomy(LRC)in our hospital.A comparison was made between the two groups in terms of tumor clinical staging,pathological grading,operative time,intraoperative blood loss,postoperative stay,number of dissected lymph nodes,lymph node positivity rate,and perioperative complications..Results:Compared with LRC,RARC had less intraoperative blood loss[(210.0±170.0)ml vs.(515.7±401.5)ml,P<0.001],shorter operative time[(344.8±121.2)min vs.(466.2±119.5)min,P<0.001]andmore lymph nodes[(18.2±8.7)vs.(12.3±6.2),P<0.001].No significant difference had been found in post-operative stays and perioperative complications between the two groups.Conclusion:Compare to LRC,patients can benefit from RARC for less intraoperative blood loss,more lymph node dissection and shorter operative time.
