摘要
目的讨论冠心病合并房颤抗凝与双联抗栓的治疗效果。方法将医院2012年10月—2014年10月期间收治的80例冠心病合并房颤患者随机分为对照组(双联抗栓治疗)与研究组(抗凝治疗)各40例,评判标准:随访情况;治疗后的CHADS2及HAS-BLED评分。结果研究组使用PPI(proton pump inhibitor,质子泵抑制剂)机率(15.0%)明显低于对照组(40.0%),主要终点事件发生率(5.0%)明显低于对照组(22.5%),差异明显(P<0.05);两组出血事件发生机率一样,且次要终点事件之间无明显差异(P>0.05)。两组患者治疗后的CHADS2评分统计学差异不明显(P>0.05);研究组患者治疗后的HAS-BLED评分明显高于对照组(P<0.05)。结论抗凝治疗将有效降低冠心病合并房颤患者PPI使用机率及主要终点事件发生机率,但需要控制好出血风险。
Objective To discuss the therapeutic effect of anticoagulation and double antithrombotic therapy in patients with coronary heart disease and atrial fibrillation.Methods 80 patients with coronary heart disease and atrial fibrillation from October 2012 to October 2014 were randomly divided into control group(double antithrombotic therapy)and study group(anticoagulant therapy)with 40 cases each.Evaluation criteria:followup;CHADS2 and has-bled scores after treatment.Results The incidence of PPI(15.0%)in the study group was significantly lower than that in the control group(40.0%),and the incidence of major end-point events(5.0%)was significantly lower than that in the control group(22.5%)(P<0.05);the incidence of bleeding events was the same in the two groups,and there was no significant difference between the secondary end-point events(P>0.05).There was no significant difference in CHADS2 score between the two groups(P>0.05),and the has-bled score of the study group was significantly higher than that of the control group(P<0.05).Conclusion Anticoagulation therapy can effectively reduce the probability of PPI use and the incidence of major end point events in patients with coronary heart disease and atrial fibrillation,but the risk of bleeding needs to be controlled.
