摘要
目的探讨厄贝沙坦氢氯噻嗪联合瑞舒伐他汀治疗血脂正常老年高血压的临床疗效及其对动脉粥样硬化的影响。方法选取会昌县中医院2017年1月—2018年1月收治的血脂正常老年高血压患者112例,采取随机数字表法分为对照组与观察组,各56例。对照组予以厄贝沙坦氢氯噻嗪与安慰剂治疗,观察组予以厄贝沙坦氢氯噻嗪联合瑞舒伐他汀治疗。两组均连续用药12个月。比较两组血压(舒张压、收缩压)、血脂指标〔总胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL-C)与低密度脂蛋白胆固醇(LDL-C)〕、超敏C反应蛋白(hs-CRP)、颈动脉内膜中层厚度(IMT)、肱动脉内皮依赖性舒张功能(EDD)与硝酸甘油介导非内皮依赖的舒张功能(NMD),并观察两组不良反应发生情况。结果两组舒张压、收缩压、TG、HDL-C、NMD比较,差异无统计学意义(P>0.05);观察组TC、LDL-C、hs-CRP、IMT低于对照组,EDD高于对照组(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论厄贝沙坦氢氯噻嗪联合瑞舒伐他汀治疗血脂正常老年高血压的临床疗效确切,可有效控制血压、改善血脂,预防动脉粥样硬化,且安全性较高。
Objective To discuss the clinical effect of irbesartan hydrochlorothiazide combined with rosuvastatin in the treatment of elderly hypertension with normal blood lipids and its impact on atherosclerosis.Methods A total of 112 cases of patients with elderly hypertension with normal blood lipids were selected from January 2017 to January 2018 in Huichang County Hospital of Traditional Chinese Medicine,which were randomly divided into control group and observation group according to the random number table method,56 cases in each group.The control group was treated with irbesartan hydrochlorothiazide and placebo,the observation group was treated with irbesartan hydrochlorothiazide and rosuvastatin.Blood pressure(diastolic pressure,systolic pressure),blood lipid index(TC,TG,HDL-C and LDL-C),hs-CRP,IMT,EDD and NMD were compared between the two groups,and the incidence of adverse reactions were observed.Results There was no significant differences in DBP,SBP,TG,HDL-C and NMD between the two groups(P>0.05);TC,LDL-C,hs CRP and IMT in the observation group were lower control group,and EDD was higher than control group(P<0.05).Conclusion Irbesartan hydrochlorothiazide combined with rosuvastatin have an exact clinical effect for elderly hypertension with normal blood lipids,it can effectively control blood pressure,improve blood lipid,prevent atherosclerosis,and with high safety.
