摘要
目的探讨胸腔镜肺叶切除术、胸腔镜肺段切除术对早期非小细胞肺癌的疗效、肺功能及远期生存质量的影响。方法以2017年4月—2018年4月广安市人民医院收治的80例早期非小细胞肺癌患者为对象,按电脑数字表法随机分为两组,各40例。A组行胸腔镜肺叶切除术治疗,B组予以胸腔镜肺段切除术治疗,比较两组疗效、肺功能及远期生存质量。结果B组手术用时较A组长,胸腔引流量、住院时长较A组少,差异有统计学意义(P<0.05);两组术中出血量、1年复发率、死亡率差异无统计学意义(P>0.05);术后1年,B组用力肺活量、第1秒用力呼气容积、肺癌患者生存质量量表评分均较A组高,差异有统计学意义(P<0.05)。结论在早期非小细胞肺癌治疗中,胸腔镜肺叶切除术、胸腔镜肺段切除术效果相当,但胸腔镜肺段切除术对患者肺功能、远期生存质量的影响更小。
Objective To investigate thoracoscopic lobectomy,thoracoscopic segmentectomy in the patients with early stage non-small cell lung cancer and its influence on the efficacy,lung function and long-term quality of life.Methods 80 cases of patients with early stage non-small cell lung cancer were admitted to Guang’an People’s Hospital during April 2017 to April 2018 as the objects,all of these patients were randomly divided into two groups according to the computer digital table method,40 cases of patients in each group.Group A was treated with thoracoscopic lobectomy,and group B was treated with thoracoscopic segmentectomy.The efficacy,lung function and long-term quality of life were compared between the two groups.Results The operation time of group B was longer than that of the group A,the length of chest drainage and hospitalization was less than that of the group A,with statistically significant difference(P<0.05).There was no statistical difference in intraoperative blood loss,1-year recurrence rate and mortality between these two groups(P>0.05).One year after operation,in the group B,the strength of vital capacity,the forced expiratory volume in the first second and the quality of life scale of lung cancer patients were higher than those in the group A,with statistically significant difference(P<0.05).Conclusion In the treatment of early nonsmall cell lung cancer,thoracoscopic lobectomy and thoracoscopic segmental resection are equally effective,but thoracoscopic segmentectomy has less effect on lung function and long-term quality of life.
