摘要
目的分析高频彩色多普勒超声在常规体检中对健康人的小肾癌的前期诊断价值。方法选取2018年3月至2020年6月辽宁电力中心医院进行常规体检中发现的肾脏占位性病变的42例患者作为研究对象,以彩色多普勒超声诊断仪对患者的腹部行全方位的扫查,观察占位性组织生理特征和病理特征,将诊断结果与病理切片组织进行对比,分析高频彩色多普勒超声诊断小肾癌的灵敏度、特异度、准确性。结果高频彩色多普勒超声对患者肾脏进行全方位、多切面的扫查后,诊断为小肾癌阳性37例、阴性5例;对42例肾脏占位性病变患者行肾穿刺活检术,对肿瘤组织切片进行病理分析中,诊断为小肾癌阳性39例、阴性3例,由此可知,高频彩色多普勒超声的诊出率88.10%(37/42)、病理切片诊出率为92.85%(39/42),高频彩色多普勒超声的诊断灵敏度为94.87%(37/39)、特异度为80%(3/5);在对42例肾脏占位性病变患者行高频彩色多普勒超声检查中发现,肾脏形状局部发生改变16例、占位组织边缘平滑13例、病变部位高回声12例、低回声7例、肿瘤内部血流呈星点状9例;肿瘤周围血流呈不规则环形或半环形13例。结论高频彩色多普勒超声下,肾脏内的占位组织的生理特性和病理特性均有较为明显地反映形式;经高频彩色多普勒超声的检查,诊断小肾癌的结果与病理分析具有较高的一致性,诊断准确率高,应用于临床中可明显提高无症状小肾癌患者的诊出率,为后续的诊疗提供可靠依据。
Objective To analyze the value of high-frequency color ultrasound in the early diagnosis of small renal carcinoma in healthy people during routine physical examination.Methods 42 patients with renal occupying lesions found during routine physical examination at Liaoning Electric Power Central Hospital from March 2018 to June 2020 were selected as the research objects.The patient’s abdomen was scanned by color Doppler ultrasound in a full range to observe the physiological and pathological characteristics of the space-occupying tissue,compare the diagnosis results with the pathological slices,and analyze the diagnosis of high-frequency color Doppler ultrasound.Sensitivity,specificity and accuracy of kidney cancer.Results Thirty-seven positive cases and five negative cases were diagnosed as small renal carcinoma after all-directional and multi-section scanning of the kidney by high-frequency color ultrasound.42 cases of renal space-occupying lesions in patients with renal biopsy and pathological analysis of tumor tissue section,the diagnosis of small renal carcinoma positive 39 cases,negative in 3 cases,therefore,the diagnosis rate of high frequency colour to exceed 88.10%(37/42),pathological diagnosis rate was 92.85%(39/42),high frequency colour to exceed the diagnostic sensitivity was 94.87%(37/39),80%(3/5);In 42 patients with renal space-occupying lesions,the results of high-frequency ultrasonography showed that the local renal shape was changed in 16 cases,the edge of the space-occupying tissue was smooth in 13 cases,the lesion site was hyperechoic in 12 cases,the hypoechoic in 7 cases,and the blood flow in the tumor was stellate in 9 cases.The blood flow around the tumor was irregular ring or semi-ring in 13 cases.Conclusion Under high-frequency color ultrasound,the physiological and pathological characteristics of the renal space occupying tissue are obviously reflected.Through high-frequency color ultrasound examination,the results of diagnosis of small renal carcinoma are consistent with pathological analysis,and the diagnosis accuracy is high.Application in clinical practice can significantly improve the diagnosis rate of asymptomatic small renal carcinoma patients,and provide reliable basis for the follow-up diagnosis and treatment.
