摘要
目的:观察两种不同型号和质量的动静脉内瘘穿刺针对血液透析患者血流量、透析器凝血程度及穿刺部位异常反应等临床治疗效果的影响。方法:将68例患者分为治疗组34例,采用NIPRO,HC-30W型穿刺针进行维持性血液透析;对照组34例,采用PERFECT,D09641型穿刺针进行维持性血液透析;每位患者连续观察10次(总680次)透析过程,对两组患者的透析器凝血程度、穿刺点渗血、局部红肿、硬结反应及血流不足引起的空抽现象进行观察和护理,并比较两组间的差异。结果:NIPRO,HC-30W型穿刺针组(治疗组),其透析器凝血程度及相应的透析器容量下降率较PERFECT,D09641型穿刺针组(对照组)有非常显著性差异(P<0.01);穿刺点组织损伤程度及穿刺点渗血、局部红肿、硬结瘙痒反应方面,两组有显著性差异(P<0.05);但在血流量方面无显著差异(P>0.05)。结论:在不影响血流量的前提下(两者均为16G针头),尽量使用内径(针头与塑料套管连接处)光滑、细小的、质量较好的动静脉内瘘穿刺针(NIPRO,HC-30W型),能明显减轻透析器凝血程度,减轻瘘管局部反应,提高透析效果。
Objective: To observe the impact of two types of A-V fistula needle in different quality on blood flowing volume in hemodialysis patients,the degree of dialyzer clotting and abnormal reactions on puncture site.Methods: 68 patients were divided into treatment and control groups at random(34 cases for each group).Type NIPRO and HC-30W needles were used to give the patients maintenance hemodialysis in treatment group;type PERFECT and D09641 needles were used for maintenance hemodialysis in control group.The process of dialysis for each patient was continuously observed for 10 times(totaling 680 times) and the observation was made on the degree of dialyzer clotting,oozing of blood on puncture point,local swelling,induration response and the vacant pumping caused by deficient blood flow,the differences between the two groups were compared.Results: There were significant differences in the comparison of the degree of dialyzer clotting and the corresponding decline in the rate of dialysis capacity between the two groups(P<0.01);the significant differences were also found in the degree of tissue damage and oozing of blood on puncture points,local swelling and itching reaction of callosity between the two groups(P<0.05);but there was no significant difference in the blood flowing volume between the two groups(P>0.05).Conclusion: A-V fistula needle with sleek internal diameter and high quality like type NIPRO and HC type-30W should be the first choice if the blood flowing volume is not affected.The needle can significantly reduce the degree of dialyzer clotting,local reactions so as to enhance the effects of hemodialysis.
