超声弹性对比指数对乳腺癌腋窝淋巴结良恶性鉴别诊断的应用价值

Value of ultrasound elasticity contrast index in the differential diagnosis of benign and malignant axillarylymph nodes of patients suffered from breast cancer

摘要 目的探讨超声弹性对比指数(elasticity contrast index,ECI)对乳腺癌腋窝淋巴结良恶性的鉴别诊断价值方法选取2016年5月—2019年10月本院收治的乳腺癌患者60例共107枚腋窝淋巴结,行超声检查,记录淋巴结的短径、深度和淋巴结距离腋动脉的距离,并行超声弹性成像,记录ECI值。以病理作为最终诊断结果,运用受试者工作特征曲线(ROC)评价其ECI值的检出效能。对恶性淋巴结组的无关变量运用Logistic回归分析,以明确影响ECI值的相关参数。结果107枚淋巴结中,恶性淋巴结为70枚,良性淋巴结为37枚,恶性淋巴结组的ECI值为(4.13±1.41),良性淋巴结组为(2.54±0.81),ECI的临界值为2.73,差异均具有统计学意义(P<0.01)。弹性成像诊断恶性淋巴结的准确性与淋巴结到腋动脉距离显著相关(OR=1.24,P<0.01)。结论ECI值在乳腺癌腋窝淋巴结良恶性的鉴别方面具有重要价值,可作为常规二维超声的重要补充。 Objective To explore the value of ultrasound elasticity contrast index(ECI)in the differential diagnosis of benign and malignant axillary lymph nodes of patients suffered from breast cancer.Methods A total of 107 lymph nodes from 60 patients those who treated in our hospital from May 2016 to October 2019 were recruited and received ultrasonography.Short diameter,depth and distance to the axillary artery were recorded,and the elastography was performed.Pathological results was set as the final point,the receiver operating characteristic(ROC)curve was used to evaluate the detect efficiency of elastography.Logistic regression analysis was carried out on unrelated variables of malignant lymph node group to clarify the relevant parameters that affect the ECI value.Results Of the 107 lymph nodes,70 were malignant,and the other 37 were benign.The ECI value of malignant lymph node group was(4.13±1.41),and that of benign lymph node group was(2.54±0.81).The optimal cutoff was 2.73.These differences were statistically significant(P<0.01).The diagnostic accuracy of elastography in diagnosing malignant lymph nodes was significantly associated with a nodule’s distance to the axillary artery(OR=1.24,P<0.01).Conclusions ECI value is of great value in the differential diagnosis of benign and malignant axillary lymph nodes of patients with breast cancer,which could be used as an important supplement of conventional two-dimensional ultrasound.
