高频超声、经直肠腔内超声及超声造影在肛周脓肿诊断中的应用 被引量:5

The application of high frequency ultrasound, transrectal ultrasound and contrast-enhanced ultrasound in diagnosis of perianal abscesses

摘要 目的探讨高频超声、经直肠腔内超声及超声造影在肛周脓肿诊断中的应用价值。方法选择2017年9月—2020年3月本院收治的82例肛周脓肿患者作为研究对象,所有患者均行高频超声检查,其中33例行经直肠腔内超声检查,21例行超声造影检查,在高频超声及经直肠腔内超声下对肛周脓肿进行术前分型,并在三种条件下分别判定脓肿的内口。结果肛周脓肿的超声分型与手术结果对比一致性较好(Kappa=0.825,P<0.001)。高频超声判断内口的敏感度为57.6%,特异度为93.3%,准确度为64.9%。经直肠腔内超声判断内口的敏感度为55.6%,特异度为100.0%,准确度为63.6%。超声造影判断内口敏感度为93.8%,特异度为40.0%,准确度为81.0%。结论高频超声及经直肠腔内超声能较准确地对肛周脓肿进行术前分型,超声造影可能提高脓肿内口的显示率,为临床治疗决策提供依据。 Objective To explore the clinical application value of high frequency ultrasound,transrectal ultrasound(TRUS)and contrast-enhanced ultrasound(CEUS)in diagnosis of perianal abscesses.Methods Eighty-two patients suffered with perianal abscesses ever admitted to our hospital during September 2017 and March 2020 were enrolled as study subjects.High frequency ultrasound was performed in all patients,while TRUS was performed in 33 cases and CEUS was performed in 21 cases.Preoperative classification was performed under high frequency ultrasound and transrectal ultrasound.Also,we tried to identify the internal openings of abscesses in all methods.Results The good consistency of classification between ultrasonography and surgery has been obtained(Kappa=0.825,P<0.001).The diagnostic sensitivity,specificity and accuracy of high frequency ultrasound in estimating internal opening were 57.6%,93.3%and 64.9%,and these for TRUS were 55.6%,100.0%and 63.6%respectively,and 93.8%,40.0%and 81.0%in CEUS.Conclusions High frequency ultrasonography and TRUS could classify the perianal abscesses accurately,while CEUS couldimprove the display rate of internal opening of perianal abscesses,all the three methods could provide basis for clinical decision making.
