摘要
目的探讨血清腺苷脱氨酶(ADA)水平与肝吸虫性胆管炎的相关性。方法收集2020年1-9月广西肝吸虫高发区县人群的体检资料,根据超声影像提示典型胆管炎表现结合流行病学史,且肝吸虫抗体阳性者为肝吸虫性胆管炎组(n=208),纳入同期体检的正常人群为对照组(n=1015)。测定分析两组间各生化指标的差异。结果与对照组比较,肝吸虫性胆管炎组血清ADA、白蛋白(ALB)、前清蛋白(PA)显著降低,单胺氧化酶(MAO)、γ谷氨酰转移酶(γ-GT)显著升高,差异均有统计学意义(P<0.05)。Logistic回归分析显示,ADA、ALB是罹患肝吸虫性胆管炎的独立危险因素(OR=0.899、0.836,95%CI:0.843~0.958、0.778~0.898;P<0.05)。结论血清ADA、ALB是罹患胆管炎的独立危险因素,可作为肝吸虫流行区域的生化监测指标,为评估医学干预提供参考。
Objective To investigate the correlation between serum adenosine deaminase(ADA)level and clonorchiasis cholangitis.Methods The physical examination data of the population in the high incidence district of Clonorchiasis sinensis in Guangxi from January to September 2020 were collected.According to the typical manifestations of cholangitis and the epidemiological history,the patients with clonorchiasis antibody positive were the clonorchiasis cholangitis group(n=208),and the normal people included in the physical examination at the same time were the control group(n=1015).The differences of biochemical indexes between the two groups were analyzed.Results Compared with the control group,the levels of ADA,albumin(ALB)and prealbumin(PA)in clonorchiasis cholangitis group were significantly lower(P<0.05);and monoamine oxidase(MAO)andγglutamyltransferase(γ-GT)increased significantly(P<0.05).Logistic regression analysis showed that ADA and ALB were independent risk factors for clonorchiasis cholangitis(OR=0.899,0.836;95%CI:0.843-0.958,0.778-0.898;P<0.05).Conclusion Serum ADA and ALB were independent risk factors for clonorchiasis cholangitis,and could be used as a biochemical monitoring index in the epidemic area of Clonorchis sinensis,and a reference for evaluating medical intervention.
