肺癌合并肺部感染患者病原菌分布及血清lncRNA NEAT1和microRNA-31表达变化 被引量:6

Distribution of pathogens and expression of serum lncRNA NEAT1 and microRNA-31 of lung cancer patients complicating lung infection

摘要 目的研究肺癌合并肺部感染患者病原菌分布及血清长链非编码RNA核富集转录体1(NEAT1)、微小RNA-31(miR-31)表达变化。方法选取河北北方学院附属第一医院2018年3月-2021年10月收治的98例肺癌合并肺部感染患者作为实验组(n=98),同时选取98例肺癌未合并肺部感染患者作为对照组(n=98)。分析实验组病原菌感染情况,比较两组血清NEAT1、miR-31表达水平。比较实验组轻度、中度、重度感染及不同临床病理特征患者血清NEAT1、miR-31表达情况,同时以Spearman相关性分析血清NEAT1、miR-31与患者肺部感染程度的相关性。结果98例合并肺部感染的患者共分离112株病原菌,其中革兰阴性菌66株占58.92%,主要为大肠埃希菌和肺炎克雷伯菌;革兰阳性菌30株占26.78%,主要以金黄色葡萄球菌和溶血葡萄球菌为主;真菌16株占14.28%,主要以白假丝酵母菌为主。与对照组比较,实验组患者血清NEAT1(1.12±0.28)vs.(4.29±0.35)(t=70.013,P<0.01)和miR-31(1.02±0.21)vs.(4.75±0.38)(t=85.048,P<0.01)表达均显著升高,差异均有统计学意义。与轻度感染者比较,中度感染和重度感染者血清NEAT1和miR-31表达水平更高(NEAT1:t_(中度vs.轻度)=8.007、t_(重度vs.轻度)=14.672,miR-31:t_(中度vs.轻度)=4.982、t_(重度vs.轻度)=22.268,P均<0.05);与中度感染者比较,重度感染者血清NEAT1和miR-31表达水平更高,差异均有统计学意义(t_(NEAT1)=10.082、t_(miR-31)=19.710,P均<0.05);TNMⅢ-Ⅳ期和肿瘤长径≥3 cm患者血清NEAT1和miR-31表达显著高于TNMⅠ-Ⅱ期和肿瘤长径<3 cm患者(NEAT1:t_(TNM分期)=14.901、t_(肿瘤长径)=30.125,miR-31:t_(TNM分期)=41.971、t_(肿瘤长径)=37.489,P均<0.05)。相关性分析结果发现,肺癌合并肺部感染患者血清NEAT1和miR-31表达与肺部感染程度成正相关性(r_(NEAT1)=0.772、r_(miR-31)=0.758,P均<0.05)。结论肺癌合并肺部感染患者病原菌主要以革兰阴性菌为主,患者血清NEAT1和miR-31表达存在明显升高,其水平变化与肿瘤的分期、进展以及感染程度相关。 Objective To investigate the distribution of pathogens and expression of serum long non-coding RNA nuclearenriched abundant transcript 1(LncRNA NEAT1)and microRNA-31(miR-31)of lung cancer patients complicating lung infection.Methods Ninety-eight patients with lung cancer complicating lung infection in our hospital from March 2018 to October 2021 were enrolled as experimental group,meantime,another 98 lung cancer patients without lung infection were set as control group.The bacterial pathogens causing infection within experimental group was analyzed.Serum levels of NEAT1 and miR-31 were detected,and compared between two groups,and among infected patients of different infection degree and clinicopathological characteristics.Meantime,the correlation of serum NEAT1 and miR-31 levels with the degree of lung infection was analyzed by Pearson correlation analysis.Results A total of 112 strains of pathogens were isolated from 98 lung cancer patients complicating lung infection.There were 66 strains of gram-negative bacteria,accounting for 58.92%,mainly Escherichia coli and Klebsiella pneumoniae,30 strains of gram-positive bacteria,accounting for 26.78%,mainly Staphylococcus aureus and Staphylococcus haemolyticus,and 16 strains of fungi,accounting for 14.28%,mainly Candida albicans.Compared with control group,serum NEAT1 and miR-31 expressions were significantly elevated in experimental group(t_(NEAT1)=70.013,t_(miR-31)=85.048,all P<0.05).Serum levels of NEAT1 and miR-31 were obviously increased in patients of moderate and severe degrees compared with patients of mild degree(NEAT1:t_(moderate vs.mild)=8.007,t_(severe vs.mild)=14.672;miR-31:t_(moderate vs.mild)=4.982,t_(severe vs.mild)=22.268;all P<0.05).Patients of severe degree had higher serum NEAT1 and miR-31 levels compared with patients of moderate degree(t_(NEAT1)=10.082,t_(miR-31)=19.710,all P<0.05).In terms of different clinicopathological characteristics,expression levels of serum NEAT1 and miR-31 in patients of TNM stageⅢ-Ⅳand tumor length≥3 cm were significantly higher than those of TNM stageⅠ-Ⅱand tumor length<3 cm,with statistical difference(NEAT1:t_(TNM stage)=14.901,t_(tumor diameter)=30.125;miR-31:t_(TNM stage)=41.971,t_(tumor diameter)=37.489,all P<0.05).Correlation analysis revealed that serum NEAT1 and miR-31 expression levels were positively correlated with the degree of lung infection in patients with lung cancer(r_(NEAT1)=0.772,r_(miR-31)=0.758,all P<0.05).Conclusion Gram-negative bacteria were the main pathogens causing infection in lung cancer patients,and the serum levels of NEAT1 and miR-31 were abnormally elevated in patients,and the changes of two indicators were closely related to the clinical stage,tumor progression and infection degree in patients.
