SCCA、CA125、HE4、β-HCG和CYFRA21-1联合检测对宫颈癌的诊断价值 被引量:10

The diagnostic value of combined detection of SCCA,CA125,HE4,β-HCG and CYFRA21-1 in cervical cancer

摘要 目的探讨鳞状细胞癌相关抗原(SCCA)、糖类抗原125(CA125)、人附睾蛋白4(HE4)、人绒毛膜促性腺激素(β-HCG)和细胞角蛋白19可溶性片段(CYFRA21-1)联合检测在诊断宫颈癌中的价值。方法将2020年9月-2022年4月河南省肿瘤医院收治的100例宫颈癌患者纳入宫颈癌组,67例Ⅲ期宫颈上皮内瘤变(CIN3)和26例CIN2患者纳入CIN组,同期11例CIN1和86名健康妇女纳入对照组。采用电化学发光免疫分析法检测血清SCCA、CA125、HE4、β-HCG和CYFRA21-1浓度;ROC曲线分析联合SCCA、CA125、HE4、β-HCG和CYFRA21-1对宫颈癌的诊断价值。结果与CIN组和对照组比较,宫颈癌组血清SCCA、CA125、HE4、β-HCG和CYFRA21-1水平显著升高,差异均有统计学意义(P均<0.001)。宫颈癌组血清SCCA、CA125、HE4、β-HCG和CYFRA21-1阳性率均显著高于CIN组和对照组,差异均有统计学意义(P均<0.001)。5种标志物联合诊断灵敏度为97.00%、特异度为96.15%、准确度为96.57%,均高于单一指标检测。血清SCCA、CA125、HE4、β-HCG和CYFRA21-1单独和联合诊断宫颈癌的曲线下面积(AUC)分别为0.753、0.847、0.744、0.692、0.754和0.953,联合检测的AUC均高于单一指标检测。结论SCCA、CA125、HE4、β-HCG和CYFRA21-1联合检测对宫颈癌的诊断价值较高。 Objective To explore the value of combined detection of squamous cell carcinoma antigen(SCCA),carbohydrate antigen 125(CA125),human epididymis protein 4(HE4),β-human chorionic gonadotropin(β-HCG)and cytokeratin 19-fragments(CYFRA21-1)in the diagnosis of cervical cancer.Methods A total of 100 cases of cervical cancer were included in cervical cancer group,67 cases of cervical intraepithelial neoplasia 3(CIN3)and 26 cases of cervical intraepithelial neoplasia 2(CIN2)were included in CIN group,11 cases of cervical intraepithelial neoplasia 1(CIN1)and 86 cases of healthy women were included in the control group.Serum SCCA,CA125,HE4,β-HCG and CYFRA21-1 were detected by electrochemiluminescence immunoassay.Receiver operating characteristic(ROC)curve was drawn to analyze the diagnostic value of SCCA,CA125,HE4,β-HCG and CYFRA21-1 in cervical cancer.Results Compared with CIN group and control group,the serum levels of SCCA,CA125,HE4,β-HCG and CYFRA21-1 in cervical cancer group were significantly higher(P<0.001).The positive rates of serum SCCA,CA125,HE4,β-HCG and CYFRA21-1 in cervical cancer group were significantly higher than those in CIN group and control group(P<0.001).The diagnostic sensitivity(97.00%),specificity(96.15%)and accuracy(96.57%)of the combined group were significantly higher than SCCA,CA125,HE4,β-HCG and CYFRA21-1 alone group.The AUC of serum SCCA,CA125,HE4,β-HCG,CYFRA21-1 and combined detection were 0.753,0.847,0.744,0.692,0.754 and 0.953,respectively;the AUC detected by combined five serum markers were significantly higher than those of single marker.Conclusion Combined detection of SCCA,CA125,HE4,β-HCG and CYFRA21-1 was of high value in the diagnosis of cervical cancer.
