摘要
目的报告及分析1例重症疟疾,探讨血小板作为疟疾严重程度的评价指标价值,为重症疟疾的临床诊疗提供参考。方法对中国驻南苏丹维和二级医院收治的1例恶性疟原虫感染病例进行分析和相关文献复习。结果患者诊断为“重症疟疾”在院治疗,以“血小板重度减少、肝功能异常”为主要临床特征,经蒿甲醚规范抗疟治疗后,患者症状消失、血小板计数恢复正常,1周后复查发现“疟疾再燃”,继续予以青蒿琥酯规范抗疟治疗后痊愈。结论血小板可作为评价疟疾严重程度的指标,血小板严重减少的疟疾患者应按重症疟疾来管理,延长复查周期,联合使用抗疟药物,以取得满意疗效。
Objective A case of severe malaria was reported and analyzed,and platelets were used as an evaluation index for the severity of malaria to provide reference for clinical diagnosis and treatment of severe malaria.Methods A case of Plasmodium falciparum infection admitted to a Chinese peacekeeping secondary hospital in South Sudan was analyzed and related literatures were reviewed.Results The patient was diagnosed with“severe malaria”and was treated in the hospital,with“severe thrombocytopenia and abnormal liver function”as the main clinical features.After standard antimalarial treatment with artemether,the patient's symptoms disappeared and the platelet count returned to normal.After one week,the patient was reexamined.It was found that“malaria re-emergence”,continued to be treated with artesunate as a standard antimalarial treatment and recovered.Conclusions Attention should be paid to platelets as an indicator for evaluating the severity of malaria.Malaria patients with severe thrombocytopenia should be managed as severe malaria,with prolonged review cycles and combined use of antimalarial drugs to achieve satisfactory clinical efficacy.
