跟进式赋能教育联合中药封包穴位热熨对宫颈癌放化疗后恶心呕吐的影响

摘要目的:观察跟进式赋能教育联合中药封包穴位热熨对宫颈癌放化疗术后恶心呕吐的影响。方法:选取104例接受放化疗治疗的宫颈癌患者,按随机数字表法分成对照组和研究组各52例。在放化疗治疗的同时,对照组给予注射用盐酸昂丹司琼治疗,研究组给予跟进式赋能教育联合中药封包穴位热熨干预。2组均干预6周。比较2组恶心呕吐发生率、伴随症状评分、自我护理能力评定量表(ESCA)评分及不良反应发生率。结果:干预后,研究组恶心呕吐发生率38.46%,低于对照组59.62%(P<0.05)。研究组食欲不振、胸胁疼痛、胃脘痞闷评分均低于对照组(P<0.05)。2组健康知识水平、自我护理技能、自我护理责任感、自我概念评分均较干预前升高(P<0.05),研究组上述4项ESCA细则评分均高于对照组(P<0.05)。研究组未发生不良反应,对照组不良反应发生率11.54%,差异有统计学意义(P<0.05)。结论:跟进式赋能教育联合中药封包穴位热熨可有效提高宫颈癌放化疗患者的自我护理能力,缓解临床症状,降低恶心呕吐发生率,安全性好。 Objective:To observe the effect of the therapy of follow-up empowerment education combined with acupoint hot pressing with Chinese medicine packages on nausea and vomiting after radiotherapy and chemotherapy for cervical cancer.Methods:A total of 104 patients with cervical cancer receiving radiotherapy and chemotherapy were divided into the control group and the study group according to random number table method,with 52 cases in each group.At the same time of radiotherapy and chemotherapy,the control group was treated with Ondansetron Hydrochloride Injection,and the study group was given follow-up empowerment education combined with acupoint hot pressing with Chinese medicine packages.Both groups were intervened for 6 weeks.The incidence of nausea and vomiting,accompanying symptom scores,Exercise of Self-Care Agency Scare(ESCA)scores,and the incidence of adverse reactions were compared between the two groups.Results:After intervention,the incidence of nausea and vomiting in the study group was 38.46%,lower than that of 59.62%in the control group(P<0.05).The scores of poor appetite,pain in the chest and rib-side,and stomach distension and stuffiness in the study group were lower than those in the control group(P<0.05).The scores of health knowledge,self-care skills,self-care responsibility,and self-concept in both groups were increased when compared with those before intervention(P<0.05),and the scores of the above four ESCA items in the study group were higher than those in the control group(P<0.05).No adverse reactions occurred in the study group,and the incidence of adverse reactions in the control group was 11.54%,the difference being significant(P<0.05).Conclusion:The therapy of follow-up empowerment education combined with acupoint hot pressing with Chinese medicine packages can effectively improve the self-care ability of cervical cancer patients receiving radiotherapy and chemotherapy,relieve clinical symptoms,reduce the incidence of nausea and vomiting,and have great safety.
