摘要
目的:探讨熊去氧胆酸联合维生素E治疗抗肿瘤药物所致急性药物性肝损伤(ADILI)的临床疗效。方法:选取2022年8月至2023年8月收集的104例因抗肿瘤药物所致的ADILI患者,根据不同治疗方案将其分为对照组和研究组,各52例,对照组接受异甘草酸镁治疗,在此基础上,研究组接受熊去氧胆酸联合维生素E治疗,比较两组肝功能指标、炎症指标、T淋巴细胞指标及不良反应。结果:治疗后,研究组血清AST、ALT、TBiL水平均低于对照组(均P<0.05);治疗后,研究组血清TNF-α、MIF、IL-6水平及NO含量均低于对照组(均P<0.05);治疗后,研究组CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)高于对照组,CD8^(+)低于对照组(均P<0.05);两组并发症发生率(21.15%vs15.38%)比较无统计学意义(P>0.05)。结论:熊去氧胆酸联合维生素E可改善抗肿瘤药物所致ADILI患者的肝功能,还可控制其炎症反应,提高机体免疫功能,且安全性较高。
Objective:To investigate the clinical effect of ursodeoxycholic acid combined with vitamin E in the treatment of acute drug-induced liver injury(ADILI)caused by antitumor drugs.Methods:A total of 104 patients with ADILI due to antitumor drugs were selected from August 2022 to August 2023.According to different treatment plans,they were divided into control group and study group,52 cases each.The control group received magnesium isoglycyrrhizinate,on the basis of which,the study group received ursodeoxycholic acid combined with vitamin E.LIver function index,inflammation index,T lymphocyte index and adverse reaction were compared between the two groups.Results:After treatment,serum AST,ALT and TBiL levels in the study group were lower than those in the control group(all P<0.05).After treatment,the serum levels of TNF-α,MIF,IL-6 and NO in the study group were lower than those in the control group(all P<0.05).After treatment,CD3^(+),CD4^(+),CD4^(+)/CD8^(+)in the study group were higher than those in the control group,and CD8^(+)was lower than those in the control group(all P<0.05).There was no significant difference in the incidence of complications between the two groups(21.15%vs15.38%)(P>0.05).Conclusion:Ursodeoxycholic acid combined with vitamin E can improve the liver function of patients with ADILI caused by antitumor drugs,control the inflammatory response and improve the immune function,with high safety.
