摘要
目的:分析黄陈枳术汤辅助早期肠内营养支持治疗对急性胰腺炎(AP)患者T淋巴细胞亚群及前清蛋白(PAB)、淀粉酶(AMY)的影响。方法:将2019年5月~2020年5月来我院就诊的146例急性胰腺炎患者采用随机数字表法分为观察组73例(黄陈枳术汤联合早期肠内营养支持治疗)和对照组73例(单纯早期肠内营养支持治疗),比较两组患者的治疗效果、T淋巴细胞亚群及PAB、AMY水平。结果:治疗后,两组患者的临床有效率分别为95.89%和86.30%,观察组明显优于对照组(P<0.05);两组患者主症腹痛腹胀以及次症恶心呕吐、发热、黄疸、皮下出血积分均明显降低,且观察组明显优于对照组(P<0.05);两组患者血清视黄醇结合蛋白(RBP)、血清白蛋白(ALB)、PAB、总蛋白(TP)明显升高(P<0.05),且观察组明显优于对照组(P<0.05);两组患者T淋巴细胞亚群、C反应蛋白(CRP)水平均有所改善,且观察组明显优于对照组(P<0.05);两组患者淀粉酶和空腹血糖水平均有改善,且观察组明显优于对照组(P<0.05)。结论:黄陈枳术汤辅助早期肠内营养支持治疗AP可以明显改善患者临床症状,增强患者免疫力,减轻炎症反应,对治疗急性胰腺炎效果明显。
Objective:To study and analyze the effect of Huangchen Zhizhu Decoction supplemented with early enteral nutrition support therapy on T lymphocyte subsets,prealbumin(PAB)and amylase(AMY)for patients with acute pancreatitis(AP).Methods:146 patients with acute pancreatitis who came to our hospital from May 2019 to may 2020 were randomly divided into 73 cases in the observation group(Huangchen Zhizhu Decoction combined with early enteral nutrition support treatment)and 73 cases in the controlled group(simple early enteral nutrition support treatment).The treatment effects,T lymphocyte subsets,PAB and Amy levels of the two groups were compared.Results:After treatment,the clinical effective rate with 95.89%of the observation group was significantly better than 86.30%of the controlled group(P<0.05).The scores of main symptoms of abdominal pain and abdominal distension and secondary symptoms of nausea and vomiting,fever,jaundice and subcutaneous hemorrhage were significantly reduced in the two groups,and the scores were significantly better in the observation group than those in the controlled group(P<0.05).The levels of serum retinol binding protein(RBP),serum albumin(ALB),PAB and total protein(TP)of the two groups were significantly increased(P<0.05),and the levels of the observation group were significantly better compared with those of the controlled group(P<0.05).The levels of T lymphocyte subsets and C-reactive protein(CRP)were improved in the two groups of patients,and the levels of the observation group were significantly better compared to the controlled group(P<0.05).The levels of amylase and fasting blood glucose were enhanced,and the levels of the observation group were significantly better than those of the controlled group(P<0.05).Conclusion:Huangchen Zhizhu Decoction with early enteral nutrition support can significantly improve the clinical symptoms,enhance the immunity and reduce inflammatory response in the treatment of AP,and it has a significant effect for treating acute pancreatitis.
