摘要
目的:研究三黄健齿汤对老年慢性牙周炎患者龈沟液(GCF)中高迁移率族蛋白1(HMGB1)、转化生长因子-β1(TGF-β1)及Shh蛋白、基质金属蛋白酶-8(MMP-8)的影响。方法:选取2018年1月~2020年1月中国中医科学院西苑医院收治的老年慢性牙周炎患者108例,随机分为研究组与对照组,各54例。对照组给予常规基础治疗,研究组在对照组基础上给予三黄健齿汤治疗。比较两组治疗前后牙周临床相关指标[牙周袋深度(PPD)、出血指数(BI)、附着丧失(AL)、菌斑指数(PLI)、牙龈指数(GI)]、治疗前后中医症候积分、治疗前后龈沟液中细胞因子水平(HMGB1、TGF-β1、Shh、MMP-8)、治疗前后骨代谢相关指标[碱性磷酸酶(ALP)、骨钙素(OCN)、骨保护素(OPG)],并比较两组临床疗效。结果:治疗后两组PPD、BI、AL、PLI均减小,且研究组低于对照组(P<0.001)。治疗后两组主症、次症积分均降低,且研究组低于对照组(P<0.001)。治疗后两组HMGB1、Shh、MMP-8水平均降低,且研究组低于对照组(P<0.001);治疗后TGF-β1水平升高,且研究组高于对照组(P<0.001)。治疗后两组ALP、OCN水平均降低,且研究组低于对照组(P<0.001);治疗后两组OPG水平均升高,且研究组高于对照组(P<0.001);研究组治疗总有效率高于对照组(P<0.05)。结论:三黄健齿汤对老年慢性牙周炎患者疗效显著,可有效改善牙周临床相关指标、龈沟液中细胞因子水平及骨代谢相关指标。
Objective:To study the effect of Sanhuang Jianchi Decoction on high mobility group protein 1(HMGB1),transforming growth factor-β1(TGF-β1),Shh protein and matrix metalloproteinase-8(MMP-8)in gingival crevicular fluid(GCF)for elderly patients with chronic periodontitis.Methods:From January 2018 to January 2020,108 elderly patients with chronic periodontitis were randomly divided into a study group and a controlled group,54 cases in each group.The controlled group was given conventional basic treatment,and the study group was given Sanhuang Jianchi Decoction treatment on the basis of the conventional treatment.Compared the periodontal clinical related indexes before and after treatment[periodontal pocket depth(PPD),bleeding index(BI),attachment loss(AL),plaque index(PLI),gingival index(GI)],cytokine levels in gingival crevicular fluid before and after treatment(HMGB1,TGF-β1,Shh,MMP-8),bone metabolism related indicators before and after treatment[alkaline phosphatase(ALP),osteocalcin(OCN),osteoprotegerin(OPG)],and compare the clinical efficacy of the two groups.Results:After treatment,PPD,BI,AL,and PLI in the two groups were reduced,and the study group was lower than the controlled group(P<0.001).After treatment,the scores of main symptoms and secondary symptoms of the two groups were decreased,and the score of the study group was lower than that of the controlled group(P<0.001).After treatment,the levels of HMGB1,Shh and MMP-8 in the two groups decreased,and the study group was lower than the controlled group(P<0.001),the level of TGF-β1 increased after treatment,and the study group was higher than the controlled group(P<0.001).After treatment,the levels of ALP and OCN in the two groups decreased,and the study group was lower than the controlled group(P<0.001),after treatment,the OPG levels of the two groups increased,and the study group was higher than the controlled group(P<0.001).The total effective rate of treatment in the study group was higher than that in the controlled group(P<0.05).Conclusion:Sanhuang Jianchi Decoction has a significant effect for elderly patients with chronic periodontitis.It can effectively improve periodontal clinical indicators,cytokine levels in gingival crevicular fluid and bone metabolism related indicators.
