摘要
目的:应用通督灸结合“烧山火”针刺手法,观察对虚寒型多发性骨髓瘤患者癌性疼痛的临床疗效。方法:纳入病例102例,随机数字法分为观察组及对照组,对照组采用WHO提出的三阶梯止痛治疗,观察组采用通督灸,烧山火针法配合三阶梯止痛法。治疗后观察两组的NRS疼痛评分、镇痛起效时间、生活质量评分、不良反应发生率、临床疗效评价及血清β-EP、PGE2、5-HT、NPY水平的差异。结果:总体有效率治疗组94.12%,对照组80.39%,两组间相比较,差异有统计学意义(P<0.05);观察组NRS评分、镇痛起效时间、不良反应发生率及PGE2、5-HT、NPY水平均明显低于对照组(P<0.05),观察组生活质量评分及血清β-EP水平明显高于对照组(P<0.05)。结论:通督灸结合烧山火针法应用于虚寒型多发性骨髓瘤患者癌性疼痛治疗中,可显著缓解患者疼痛,降低药物不良反应发生率,降低血清疼痛介质水平,改善患者生活质量。
Objective:To observe the clinical efficacy of auxiliary analgesia for cancer pain caused by multiple myeloma of deficiency-cold type by using Tongdu moxibustion combined with“burning fire”acupuncture technique.Methods:102 cases were enrolled and randomly divided into a treatment group and a controlled group by the random number method.The controlled group was treated with the three-step analgesic treatment proposed by WHO,and the treatment group was treated with moxibustion,burning mountain fire needle and three-step analgesia.After treatment,the differences in NRS pain numerical score,analgesic onset time,quality of life score,adverse reaction incidence,clinical efficacy evaluation,and serumβ-EP,PGE2levels were observed between the two groups.Results:The overall effective rate was 92.68%in the treatment group and 78.05%in the controlled group.The difference between the two groups was statistically significant(P<0.05);the treatment group’s NRS score,analgesic onset time,incidence of adverse reactions and PGE2The 5-HT and NPY levels were significantly lower than those of the controlled group(P<0.05).The quality of life score and serumβ-EP level of the treatment group were significantly higher than those of the controlled group(P<0.05).Conclusion:Applying Tongdu moxibustion combined with burning mountain fire needle method in the treatment of deficiency-cold type multiple myeloma with cancer pain can significantly relieve pain,reduce the incidence of adverse drug reactions,reduce serum pain mediator levels,and improve the quality of life of patients.
