摘要
目的:对针刺联合CT引导下射频热凝治疗腰椎间盘突出症(LDH)的疗效进行初步研究。方法:选取70例LDH患者为研究对象,按照随机数表法分为研究组(n=38)与对照组(n=32),对照组患者采用CT引导下射频热凝治疗,研究组在对照组基础上联合针刺治疗。比较治疗前及治疗3周后,两组患者中医症候(肢体麻木、畏寒肢冷、肢体疼痛、活动不利)积分变化;分析治疗前、治疗后1个月(T1)、治疗后3个月(T2)及治疗后6个月(T3)时,两组患者疼痛程度[视觉模拟量表(VAS)]、腰椎功能[Oswestry功能障碍指数(ODI)]评分变化。结果:治疗3周后,两组患者肢体麻木、畏寒肢冷、肢体疼痛、活动不利积分较治疗前降低(P<0.05),且研究组低于同一时间对照组(P<0.05);T1、T2、T3时,两组患者VAS、ODI评分均低于治疗前(P<0.05),且研究组低于同一时间对照组(P<0.05)。结论针刺联合CT引导下射频热凝有助于改善LDH患者临床症状,缓解其疼痛感,改善腰椎功能。
Objective:To conduct a preliminary study on efficacy of applying acupuncture combined with CT-guided radiofrequency thermocoagulation in the treatment of lumbar disc herniation(LDH).Methods:Totally 70 patients with LDH were selected as the research subjects,and they were divided into a study group(n=38)and a controlled group(n=32)according to the random number table method.The controlled group was treated with CT-guided radiofrequency thermocoagulation,and the study group was combined with acupuncture on the basis of the controlled group.Before treatment and after 3 weeks of treatment,the changes of scores of TCM syndromes(limb numbness,extreme chilliness,limbpain,poor activity)were compared between the two groups.Before treatment,at 1 month after treatment(T1),at 3 months after treatment(T2)and at 6 months after treatment(T3),the pain level[Visual Analogue Scale(VAS)]and lumbar function[Oswestry Dysfunction Index(ODI)]were analyzed in the two groups.Results:After 3 weeks of treatment,the scores of limb numbness,extreme chilliness,limb pain and poor ability in the two groups were lower than those before treatment(P<0.05),and the scores in the study group were lower than those in the controlled group at the same time point(P<0.05).At T1,T2 and T3,the scores of VAS and ODI in the two groups were lower than those before treatment(P<0.05),and the scores in the study group were lower than those in the controlled group at the same time point(P<0.05).Conclusion:Acupuncture combined with CT-guided radiofrequency thermocoagulation can help improve the clinical symptoms,relieve the pain and improve lumbar function of patients with LDH.
