摘要
目的:观察加味当归补血汤联合厄贝沙坦治疗糖尿病肾病(DN)患者的疗效及对肾功能、血清免疫因子的影响。方法:选取2017年1月~2020年1月庐江县中医院收治的80例DN患者,随机数字表法分为对照组和观察组,各40例。对照组接受厄贝沙坦治疗,观察组在对照组基础上接受加味当归补血汤治疗。对比两组患者临床疗效和治疗后中医症状积分、肾功能、免疫功能、血管内皮功能。结果:观察组临床治疗总有效率为90.0%(36/40)明显高于对照组70.0%(28/40),差异有统计学意义(P<0.05);治疗后观察组中医症状积分和24h尿蛋白定量(24hUPro)、尿清蛋白排泄率(UAER)、内皮素-1(ET-1)、可溶性细胞间粘附分子-1(sICAM-1)水平及CD3^(+)、CD8^(+)、CD4^(+)/CD8^(+)明显低于对照组,Ccr、CD4^(+)百分比明显高于对照组,差异有统计学意义(P<0.05);两组患者未出现严重不良反应。结论:加味当归补血汤联合厄贝沙坦治疗DN疗效显著,可有效调节机体免疫和血管内皮功能,改善肾功能,且安全性高,值得推广。
Objective:To investigate the effect of Modified Danggui Bushui Decoction combined with Irbesartan for treating diabetic nephropathy(DN)patients and its influence on renal function and serum immune factors.Methods:80 patients with DM treated by Lujiang County Hospital of Traditional Chinese Medicine from January 2017 to January 2020 were divided into a controlled group and an observation group,40 cases respectively according to the random number table method.The controlled group was treated with Irbesartan,and the observation group received the treatment of Danggui Buxue Decoction on the basis of the controlled group.The clinical efficacy of the two groups was compared with the symptom score,renal function,immune function and vascular endothelial function after treatment.Results:The total effective rate of clinical treatment in the observation group was 90.0%(36/40)significantly higher than that of the controlled group(28/40),and the difference was statistically significant(P<0.05).After treatment,the symptom score of the observation group and 24 h urine protein quantification(24 h Upro),urinary albumin excretion rate(UAER),endothelin-1(ET-1),soluble intercellular adhesion molecule-1(sICAM-1)level,CD3^(+),CD8^(+),CD4^(+)/CD8^(+)were significantly lower than that of the controlled group,and the percentage of CCR and CD4^(+)was significantly higher than that of the controlled group(P<0.05).There was no serious adverse reactions in the two groups.Conclusion:The combination of Modified Danggui Buxue Decoction and Irbesartan has a significant effect on DN,which can effectively regulate the immune and vascular endothelial function of the body,improve the renal function,and has high safety,which is worth popularizing.
