摘要
目的:探讨祛风除湿止痛汤联合甲氨蝶呤治疗类风湿关节炎(RA)的效果。方法:选取2021年3月~2022年3月在本院收治的86例RA患者,采用随机编号法进行分组,偶数纳入观察组,奇数纳入对照组,每组各43例。对照组采用甲氨嘌呤治疗,观察组在对照组基础上添加祛风除湿止痛汤方治疗。两组均持续治疗8周,观察两组患者临床疗效。比较两组患者治疗前后临床体征[关节疼痛度、肿胀数、晨僵时间]、中医证候积分及炎症因子[血清类风湿因子(RF)、C反应蛋白(CRP)、血沉(ESR)、肿瘤坏死因子-α(TNF-α)、白介素-10(IL-10)、白介素-35(IL-35)]变化以及治疗期间不良反应发生率。结果:观察组总有效率为90.70%,高于对照组的72.09%(P<0.05)。治疗后,观察组关节疼痛度、关节肿胀个数、晨僵时间及中医证候积分低于对照组(P<0.05);观察组RF、CRP、ESR、TNF-α水平低于对照组(P<0.05);观察组IL-10、IL-35水平高于对照组(P<0.05)。两组总不良反应率比较无差异(20.93%vs 23.26%)(P>0.05)。结论:祛风除湿止痛汤联合甲氨嘌呤可有效提高临床疗效,改善RA患者临床症状,降低炎症因子水平,较单用甲氨嘌呤获益更多。
Objective:The objective of this study is to investigate the therapeutic effect of Qufeng Chushi Zhitong Decoction combined with methotrexate on patients with rheumatoid arthritis(RA).Methods:From March 2021 to March 2022,86 patients diagnosed with RA were enrolled in this study.The patients were randomized by a computer-generated random number sequence into an observation group(n=43)and a control group(n=43).The control group received methotrexate alone,while the observation group was treated with a combination of Qufeng Chushi Zhitong Decoction and methotrexate for eight weeks.The clinical efficacy of the treatment was evaluated by observing the changes in clinical signs,TCM symptoms,and inflammatory factors.Results:The total response rate in the observation group was significantly higher than that in the control group(90.70%vs 72.09%;P<0.05).After treatment,the observation group had a lower joint pain score,fewer swelling joints,and shorter morning stiffness time than the control group(P<0.05).Additionally,the observation group had lower levels of serum rheumatoid factor(RF),C-reactive protein(CRP),erythrocyte sedimentation rate(ESR),and tumor necrosis factor-α(TNF-α)than the control group(P<0.05)and higher levels of interleukin-10(IL-10)and interleukin-35(IL-35)(P<0.05).The total incidence of adverse reactions was similar in both groups(20.93%vs 23.26%;P>0.05).Conclusion:The combination of Qufeng Chushi Zhitong Decoction and methotrexate can effectively improve the clinical efficacy of RA treatment,relieve clinical symptoms,and reduce the levels of inflammatory factors.
