显微手术夹闭与血管内介入栓塞术对大脑中动脉瘤破裂患者神经功能影响 被引量:1

Effect of microsurgical clipping and vascular interventional embolization on nerve function in patients with middle cerebral artery aneurysm rupture

摘要 目的大脑中动脉瘤是大脑内部动脉血管异常膨突后形成的颅内动脉瘤,是造成蛛网膜下出血的首位病因。本研究探讨显微手术夹闭与血管内介入栓塞术对大脑中动脉动脉瘤破裂患者神经功能影响。方法选取温县人民医院2014-02-12-2018-03-24收治的大脑中动脉瘤破裂患者,在患者或家属知情同意前提下,采用组间性别、年龄、HuntHess分级匹配的原则,选择对照组和观察组,每组51例。对照组采用血管内介入栓塞术治疗,观察组采用显微手术夹闭治疗,分析两组神经功能及术后并发症状况。结果观察组和对照组术后2个月美国国立卫生研究院卒中量表(national institute of health stroke scale,NIHSS)评分分别为(8.21±6.34)和(8.24±6.42)分,t=0.024,P=0.981;术后4个月NIHSS评分分别为(3.78±1.22)和(3.98±1.33),t=0.791,P=0.491。观察组术后并发症发生率为15.69%,略低于对照组的19.61%,但差异无统计学意义,χ~2=0.270,P=0.603。结论大脑中动脉瘤破裂患者采用显微手术夹闭与血管内介入栓塞术进行治疗,均可获得较好的效果,术后神经功能恢复及并发症发生率相似。 OBJECTIVE The middle cerebral artery aneurysm is an intracranial aneurysm formed after the abnormal expansion of the arteries inside the brain,which is the first cause of subarachnoid hemorrhage.This study investigates the effects of microsurgical clipping and endovascular embolization on neurological function in patients with cerebral artery aneurysm rupture.METHODS Patients with cerebral aneurysm rupture admitted to Wenxian County People’s Hospital from February 12,2014 to March 24,2018 were selected.Under the premise of informed consent of the patient or family,the control group and observation group were selected according to the principle of gender,age and Hunt-Hess classification,with 51 cases in each group.The control group underwent vascular interventional embolization,and the observation group was treated with microsurgical clipping.The nerve function and postoperative complications in two groups were observed.RESULTS The national institute of health stroke scale(NIHSS)scores of the observation group and the control group after surgery for 2 months were(8.21±6.34)and(8.24±6.42),t=0.024,P=0.981,respectively.The 4-month NIHSS scores were(3.78±1.22)and(3.98±1.33),t=0.791,P=0.491.The incidence of postoperative complications in observation group(15.69%)was slightly lower than that in control group(19.61%),but the difference was not statistically significant(χ~2=0.270,P=0.603).CONCLUSION Microsurgical clipping and vascular interventional embolization in treatment of patients with middle cerebral artery aneurysm rupture can achieve better effects,the postoperative recovery of nerve function and complications are similar.
