摘要
肥胖型2型糖尿病一方面加重疾病危害,另一方面加大疾病治疗难度。为更好开展临床肥胖型2型糖尿病内科治疗,改善患者治疗效果,本研究总结其内科治疗现状。应用PubMed、中国知网和万方数据库,以"肥胖型""2型糖尿病"和"内科治疗"等为关键词,检索2009-01-2019-12相关文献。纳入标准:(1)2型糖尿病患者及体质量指数≥28kg/m2;(2)伴有高脂血症、冠状动脉粥样硬化性心脏病和脑血管疾病;(3)经过标准内科治疗和中医中药及针灸治疗成功减轻体质量,有效控制血糖。排除标准:(1)有严重并发症,且对肥胖型2型糖尿病内科治疗效果产生干扰;(2)单纯依赖内科用药、难以改变饮食及生活习惯患者;(3)在过去的10年中,参与任何药物试验者。共检索到英文文献15篇,中文文献57篇,最终纳入35篇文献。结果表明,通过控制饮食、适度运动及内科治疗后,肥胖型2型糖尿病患者血糖、糖化血红蛋白及体质量指数均得到改善。内科综合减重方案可明显改善患者血糖及胰岛素抵抗,同时可降低患者血脂水平,是安全有效的治疗方案。
On the one hand,obese type 2 diabetes aggravates the disease,and on the other hand,it makes the treatment of the disease more difficult.In order to better develop clinical medical treatment of obese type 2 diabetes and improve the treatment effect of patients,this study summarizes the current medical treatment of obese type 2 diabetes.Using Pubmed,China HowNet,and Wanfang Database,with the keywords of " obesity" ",type 2 diabetes" and " medical treatment" as the keywords,the relevant documents were retrieved from January 2009 to December 2019.Inclusion criteria:(1)patients with type 2 diabetes,body mass index≥28 kg/m2;(2)with hyperlipidemia,coronary atherosclerotic heart disease,cerebrovascular disease;(3)undergoing standard medical therapies and traditional Chinese medicine and acupuncture have successfully reduced weight and effectively controlled blood sugar.Exclusion criteria:(1)patients with severe complications that interfere with the medical treatment of obesity type 2 diabetes;(2)patients who rely solely on medical medication and have difficulty changing their diet and lifestyle;(3)participate in any drug trial in the past 10 years.A total of15 English documents and 57 Chinese documents were retrieved,and 35 documents were finally included.After controlling diet,moderate exercise,and medical treatment,the blood glucose,glycated hemoglobin,and body mass index of obese patients with type 2 diabetes have been improved.Comprehensive medical weight loss programs can significantly improve blood glucose and insulin resistance in patients,while reducing blood lipid levels.
