摘要
目的分析2016-2020年上海市闵行区社区卫生资源的配置情况及其公平性。方法通过政府公开信息、统计年鉴和访谈收集2016-2020年上海市闵行区社区卫生资源数据,对闵行区社区卫生资源包括社区执业(助理)医师、注册护士、其他卫生技术人员、床位配置等的现状和增长状况进行描述,以Lorenz曲线和Gini系数从常住人口、地理面积评价社区卫生资源配置公平性。结果2016-2020年,闵行区每千人口社区执业(助理)医师、注册护士数、其他卫生技术人员数分别增长5.50%、8.96%、18.24%,编制床位、实有床位分别减少5.01%、40.76%。社区卫生人力资源按服务人口的Gini指数为0.12~0.28,配置的公平性较高。按地理面积的Gini指数为0.31~0.45,处于相对不合理状态。按服务人口和地理面积社区实有床位的Gini系数为0.37~0.45,处于相对不合理状态。结论政府应加强社区卫生人才的引进和培养力度,增加社区床位,提高社区床位配置的公平性和可及性,完善社区卫生资源的配置。
Objective To analyze the distribution and equity of community health resources in Minhang District of Shanghai from 2016 to 2020,and to provide information and suggestions for improving the distribution of community health resources in Minhang District.Methods The data of community health resources in Minhang District of Shanghai from 2016 to 2020 were collected through government public information,statistical yearbooks and interviews to describe the current situation and growth of community health resources in Minhang District,including community practicing(Assistant)doctors,registered nurses,other health technicians,bed allocation.From the perspective of permanent population and geographical area,Lorenz curve and Gini coefficient were used to evaluate the fairness of community health resource allocation.Results From 2016 to 2020,the number of practicing(assistant)doctors,registered nurses and other health technicians per 1000 population in Minhang District increased by 5.50%,8.96%and 18.24%respectively,and the number of prepared beds and actual beds decreased by 5.01%and 40.76%,respectively.The Gini coefficient of community health human resources by serving population was between 0.12 and 0.28,and the allocation fairness was high.The Gini index by geographical area was between 0.31 and 0.45,which was in a relatively unreasonable state.The Gini coefficient of actual beds by serving population and geographical area was between 0.37 and 0.45,which was in a relatively unreasonable state.Conclusion The government should strengthen the introduction and training of community health talents,increase community beds,improve the fairness and accessibility of community bed allocation,and improve the allocation of community health resources.
