甘南藏族人群膳食模式与原发性高血压关系探讨

Relationship between dietary pattern and essential hypertension in Gannan Tibetan population

摘要 目的了解甘南藏族自治州藏族人群膳食模式,探讨藏族人群膳食模式与原发性高血压之间的关系。方法于2014-07-20-2015-03-01在甘肃省甘南藏族自治州合作市及夏河县采用多阶段抽样随机抽取甘南藏族自治州藏族高血压患者352例和非高血压藏族对照人群323人,对其进行流行病学调查及体格检查,采用因子分析法建立藏族人群膳食模式,多因素logistic回归分析探讨藏族人群膳食模式与原发性高血压之间的关系。结果经因子分析对该人群建立了4种膳食模式,分别为禽蛋咸菜膳食模式、藏族特色膳食模式、新鲜蔬果膳食模式、谷物主食膳食模式,其中猪牛羊肉、熏肉腊肉、奶及奶制品为主的藏族特色膳食模式是藏族人群原发性高血压的危险因素(OR=3.182,95%CI为1.916~5.283),禽蛋咸菜膳食模式(OR=0.298,95%CI为0.172~0.515)、新鲜蔬果膳食模式(OR=0.465,95%CI为0.269~0.804)及谷物主食膳食模式(OR=0.210,95%CI为0.124~0.357)则是藏族人群原发性高血压的保护因素。结论膳食模式与该地区原发性高血压的发生存在相关性,减少猪牛羊肉、熏肉腊肉的摄入,增加果蔬禽蛋摄入量可降低高血压发病风险。 Objective To investigate the dietary pattern of Tibetan population in Gannan Tibetan Autonomous Prefecture and to explore the relationship between dietary pattern and essential hypertension in Tibetan population.Methods From July 20,2014 to March 1,2015,352 Tibetan hypertensive patients and 323 non hypertensive Tibetan control people were randomly selected by multi-stage sampling in Hezuo City and Xiahe County of Gannan Tibetan Autonomous Prefecture in Gansu Province.Epidemiological investigation and physical examination were conducted among these people.The dietary pattern of Tibetan people was established by factor analysis.Multivariate logistic regression analysis was used to explore the relationship between the dietary pattern of Tibetan people and essential hypertension.Results There were four dietary patterns established by factor analysis,which were poultry egg and salted vegetables dietary pattern,Tibetan characteristic dietary pattern,fresh fruits and vegetables dietary pattern,grain staple food dietary pattern.Among them,Tibetan characteristic dietary pattern,mainly consisted of pork,beef,mutton,bacon,milk and milk products was the risk factor of essential hypertension in Tibetan population(OR=3.182,95%CI:1.916-5.283).The dietary patterns of poultry eggs and pickles(OR=0.298,95%CI:0.172-0.515),fresh vegetables and fruits(OR=0.465,95%CI:0.269-0.804),and grain staple food(OR=0.210,95%CI:0.124-0.357)were the protective factors of essential hypertension in Tibetan population.Conclusions Dietary patterns are correlated with essential hypertension in this area.Reducing the intake of pork,beef,mutton,bacon while increasing the intake of fruits,vegetables,poultry and eggs can reduce the risk of hypertension.
