老年住院患者营养评估及影响因素相关性研究 被引量:4

The correlation study of nutritional evaluation and influencing factors of elderly inpatients

摘要 目的探讨简易营养评估量表(MNA)在老年住院患者临床应用,并与临床生化指标进行比较,明确MNA在临床应用的可靠性。方法选取2021-05-10-2021-06-30融安县中医医院内科住院的105例≥65岁患者作为研究对象。患者符合入组标准,行MNA评分表并记录各项生化指标,依据营养风险程度分为营养不良组、营养不良风险组及营养良好组。采用SPSS 26.0统计学分析各组数据之间相关性,了解MNA在明确营养情况时的可靠性。结果住院患者营养风险发生率为70.5%(74/105),其中营养不良21例(20.0%),营养不良风险53例(50.5%),无营养风险31例(29.5%)。3组间因素比较显示,性别、身体质量指数(BMI)、淋巴细胞数(LYM)、淋巴细胞比率(LYM%)、血红蛋白(HGB)、前白蛋白(PA)、总蛋白(TP)、白蛋白(Alb)、总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL)差异显著,差异有统计学意义,P<0.05。二元logistic回归分析结果表明BMI是营养情况的独立预测因素。结论营养不良组及营养不良风险组的BMI、HGB、TP、Alb、TC、TG及LDL与营养程度相关;BMI是营养程度的独立预测因素;BMI的约登指数最大值是0.470,对应的BMI是20.305,可以作为营养不良与营养不良风险的临界值。 Objective To explore the clinical application of Mini Nutritional Assessment Scale(MNA)in elderly hospitalized patients,and compare it with clinical biochemical indicators to clarify the reliability of MNA in clinical application.Methods From May 10,2021 to June 30,2021,105 patients aged≥65 years who were hospitalized in the Internal Medicine Department of Rong’an County Traditional Chinese Medicine Hospital were selected as the research subjects.All the inpatients met the inclusion criteria,received MNA scoring and recorded various biochemical indicators.They were divided into malnutrition group,malnutrition risk group and well-nourished group according to the degree of malnutrition.SPSS 26.0 was used to statistically analyze the correlation between the data of each group,and clarify the reliability of MNA in determining nutritional status.Results The incidence of nutritional risk in hospitalized patients was 70.5%(74/105),of whom 21(20.0%)were undernourished,53(50.5%)had malnutrition risk,and 31(29.5%)had no nutritional risk.The comparison of factors among the three groups suggested that sex,body mass index(BMI),lymphocyte ratio(LYM%),hemoglobin(HGB),prealbumin(PA),total protein(TP),albumin(Alb),total cholesterol(TC),triglycerides(TG),and low-density lipoprotein cholesterol(LDL)were significantly different,and the difference was statistically significant,P<0.05.The results of binary logistic regression analysis showed that BMI was an independent predictor of nutritional status.Conclusions BMI,HGB,TP,Alb,TC,TG and LDL in malnutrition group and malnutrition risk group are related to nutritional level.BMI is an independent predictor of nutritional level;The maximum BMI of the Jordon index is 0.470,which corresponds to a BMI of 20.305,which can be used as a cut-off value for malnutrition and malnutrition risk.
