重症监护早产儿肺部感染危险因素分析与护理对策分析 被引量:1

摘要 目的分析探讨重症监护早产儿肺部感染危险因素、以及相对应的护理措施,进一步降低重症监护早产儿肺部感染情况的出现。方法此次研究选取的患者资料,为2018年1月—2019年11月期间,四川省广元市第一人民医院重症监护室中的180例早产儿,其中根据患者是否出现肺部感染为分组标准,未出现肺部感染的患者为参照组,出现肺部感染的患者为感染组,对两组患者的资料进行分析筛选,寻找患者肺部感染的危险因素,并提出护理对策。结果单因素分析结果中,具有差异的因素包括出生时体质量、胎龄、基础疾病、通气时间、吸氧时间等,差异有统计学意义(P<0.05);多元回归分析结果中,主要危险因素包括出生体质量、昏迷、以及机械通气时间3项因素,回归系数为正值,且OR>1。结论研究数据显示,重症监护早产儿出现肺部感染的主要危险因素包括出生体质量、昏迷、以及机械通气时间;掌握重症监护早产儿出现肺部感染的主要危险因素,并采取针对性的护理干预措施,可以有效降低患者出现肺部感染的几率,临床价值明显。 Objective To analyze and explore the risk factors of pulmonary infection in intensive care premature infants and corresponding nursing measures to further reduce the incidence of pulmonary infection in intensive care premature infants.Methods The patient data selected for this study were 180 premature infants in the hospital's intensive care unit from January 2018 to November 2019.Of these,the grouping criteria were based on whether the patient had a lung infection and no lung appeared.The infected patients were the reference group,and the patients with pulmonary infection were the infection group.The data of the two groups of patients were analyzed and screened to find the risk factors of the patients'lung infection,and the nursing countermeasures were proposed.Results Among the results of univariate analysis,the factors that had differences included body weight at birth,gestational age,basic diseases,ventilation time,oxygen inhalation time,etc.,with statistically significantdifference(P<0.05);The major risk factors in the results of multiple regression analysis including birth weight,coma,and mechanical ventilation time.The regression coefficient is positive,and OR>1.Conclusion The research data show that the main risk factors of lung infection in intensive care preterm infants include birth weight,coma,and mechanical ventilation time;master the main risk factors of lung infection in intensive care preterm infants,and take targeted care Intervention measures can effectively reduce the probability of lung infection in patients,and its clinical value is obvious.
