摘要
目的研究临床麻醉与术后镇痛中应用舒芬太尼及右美托咪定的效果。方法选择2019年9月—2020年8月江苏省张家港市第五人民医院收治的92例接受全身麻醉手术患者为研究对象,采用随机数表法分成对照组和试验组,每组46例。对照组术中予芬太尼麻醉,术后予舒芬太尼镇痛;试验组术中予舒芬太尼麻醉,术后予舒芬太尼+右美托咪啶镇痛。比较两组麻醉恢复指标、疼痛视觉模拟评分法(VAS)评分和副反应发生率。结果试验组气管导管拔除时间(8.26±3.01)min、苏醒时间(27.63±5.20)min及自主呼吸恢复时间(8.99±1.95)min短于对照组(12.96±3.48)min、(38.02±5.17)min、(12.26±2.68)min,差异有统计学意义(t=6.928,P<0.05;t=9.610,P<0.05;t=6.692,P<0.05);对照组术后2 h VAS评分(3.72±1.30)分、24 h VAS评分(2.35±0.98)分、48 h VAS评分(0.69±0.34)分高于试验组(2.45±1.20)分、(1.09±0.63)分、(0.32±0.29)分,差异有统计学意义(t=4.869,P<0.05;t=7.335,P<0.05;t=5.616,P<0.05);试验组副反应总发生率2.17%低于对照组13.04%,差异有统计学意义(χ2=3.866,P<0.05)。结论舒芬太尼和右美托咪啶在临床麻醉、术后镇痛中的应用效果确切,临床推广价值高。
Objective To study the effect of sufentanil and dexmedetomidine in clinical anesthesia and postoperative analgesia.Methods 92 patients with general anesthesia in the hospital from September 2019 to August 2020 were randomly divided into control group and experimental group,with 46 cases in each group.The control group was given fentanyl anesthesia and postoperative sufentanil analgesia.The experimental group was given sufentanil anesthesia during operation and sufentanil+dexmedetomidine analgesia after operation.Anesthesia recovery index,visual analogue scale(VAS)score and incidence of side effects were compared between the two groups.Results The extubation time,recovery time and spontaneous breathing recovery time of the experimental group were(8.26±3.01)min,(27.63±5.20)min and spontaneous breathing recovery time of the experimental group(8.99±1.95)min were lower than(12.96±3.48)min、(38.02±5.17)min、(12.26±2.68)min of the control group,with signifficant difference(t=6.928,P<0.05;t=9.610,P<0.05;t=6.692,P<0.05).The 2 h VAS score(3.72±1.30),24 h VAS score(2.35±0.98),and 48 h VAS score(0.69±0.34)of the control group were higher than those of(2.45±1.20)and(1.09±0.63)points,(0.32±0.29)points in the experimental group.The total incidence of side effects in the experimental group was 2.17%,which was lower than 13.04%in the control group,with statistically significant difference(χ2=3.866,P<0.05).Conclusion Sufentanil and dexmedetomidine in clinical anesthesia,postoperative analgesia application effect is exact,clinical promotion value is high.
