摘要
目的探讨急性加重史对嗜酸性粒细胞(eosinophils,EOS)型慢性阻塞性肺疾病(简称慢阻肺)患者重度急性加重(acute exacerbations,AE)风险、肺炎发生风险、口腔念珠菌感染等方面的影响。方法纳入于2019年8月至2020年10月南华大学附属长沙中心医院住院的血EOS≥300/μL的慢阻肺急性加重期患者52例。根据过去一年是否有≥1次急性加重住院史,分为急性加重组(29例)和非急性加重组(23例)。随访1年,对比分析两组患者在慢阻肺评估测试(COPD assessment test,CAT)改善方面、年重度急性加重次数、肺炎发生次数及口腔念珠菌感染的情况。结果急性加重组患者在年重度急性加重风险、肺炎发生风险、口腔念珠菌感染方面均高于非急性加重组,差异均具有统计学意义(P<0.05)。两组患者在CAT评分改善方面无统计学差异(P>0.05)。结论对于血EOS≥300/μL的慢阻肺患者,既往急性加重史在预测血EOS型慢阻肺患者再发急性加重、肺炎发生及口腔念珠菌感染等方面具有重要的价值。
Objective To investigate the influence of acute exacerbation history on the annual risk of severe acute exacerbation(AE),pneumonia and oral candida infection in patients with eosinophilic(EOS)chronic obstructive pulmonary disease(COPD).Methods A total of 52 patients with acute exacerbation of COPD with blood EOS≥300/μL were enrolled in Changsha Central Hospital affiliated to University of South China from August 2019 to October 2020.The patients were divided into acute plus recombination(29 cases)and non-acute plus recombination(23 cases)according to whether they had been hospitalized for acute exacerbation≥1 time in the past year.Patients were followed up for 1 year,and the improvement of COPD assessment test(CAT),the number of severe acute exacerbations,pneumonia and oral candida infection were compared and analyzed between the two groups.Results The annual risk of severe acute exacerbation,pneumonia and oral candida infection in patients with acute recombination were higher than those without acute recombination,and the differences were statistically significant(P<0.05).There was no statistical difference in CAT score improvement between 2 groups(P>0.05).Conclusions The history of acute exacerbation is important in predicting the recurrence of acute exacerbation,pneumonia and oral candida infection in patients with chronic obstructive pulmonary disease(COPD)with blood EOS≥300/μL.
