摘要
病原体随血流致全身多处出现感染灶,临床表现多样,某些部位起病隐匿,治疗方案需考虑药物在各组织中的有效浓度、给药方式及治疗周期。本文介绍了1例确诊金黄色葡萄球菌血流感染患者,其同时出现肺部、泌尿系、颅内及眼内感染,属临床少见,通过分析患者的临床表现、治疗以及转归,积累诊治经验,希望引起临床重视。本例患者眼内炎反复,万古霉素玻璃体腔内注射控制不佳,最终行眼内容物剜除术,其余部位感染在万古霉素为主的抗感染治疗下痊愈。
The pathogen travels through the bloodstream leading to multiple foci of infection throughout the body.The clinical manifestations of this disease are diverse,with occult onset in some parts.The treatment plan should consider the effective concentration of the drug in each tissue,the mode of administration and the duration of treatment.This paper introduces a case of Staphylococcus aureus bloodstream infection,which is rare in clinical practice,with simultaneous pulmonary,urinary,intracranial and intraocular infections.Through the analysis of the clinical manifestations,treatment and outcome of the patients,we accumulated experience in diagnosis and treatment and hope to attract clinical attention.In this case,the patient had recurrent endophthalmitis,and the intravitreal injection of vancomycin was not effective.Finally,enucleation of the eye contents was performed,and the rest of the infection was cured by anti-infective therapy mainly with vancomycin.
